RESOLUTION APPROVING AN ADDENDUM TO
AGREEMENT NO. 1038172 WITH UNIFIRST, OF CHICAGO,
ILLINOIS, ORIGINALLY EXECUTED SEPTEMBER 6, 2022,
AND SET TO EXPIRE ON NOVEMBER 20, 2027, TO ADD
REQUIRED PPE SHIRT ITEM NO. 03HKHT FR CAT 2 ATPV
8.3, ANSI CLASS 3 AT $3.25 EACH PER WEEK PER
WEARER, IN CONJUNCTION WITH THE UNIFIRST
UNIFORM AGREEMENT, CONTRACT NO. 2491,
RESOLUTION APPROVING AN ADDENDUM TO THE
UNIFIRST UNIFORM AGREEMENT, CONTRACT NO. 2491
TO ADD REQUIRED PPE ITEMS, AND AUTHORIZING THE
MAYOR TO EXECUTE SAID DOCUMENT.
WHEREAS, the UniFirst Uniform Agreement, Contract No. 2491 was originally
executed on September 6, 2022, to provide the Public Works and Waste Management
departments with work clothing and office supplies, and
WHEREAS, the the UniFirst Uniform Agreement, Contract No. 2491 is set to expire on
November 20, 2027, and
WHEREAS, the original contract needs to be amended to add required PPE shirt item
No. 03HKHT FR Cat 2 ATPV 8.3, ANSI Class 3 at $3.25 each per week per wearer for the
Traffic Department.
NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
WATERLOO, IOWA, that the addendum to the UniFirst Uniform Agreement, Contract No.
2491 to add required PPE shirt item No. 03HKHT FR Cat 2 ATPV 8.3, ANSI Class 3 at $3.25
each per week per wearer for the Traffic Department, is hereby approved, and the Mayor is
authorized and directed to execute said document on behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 1st day of July 2024.
