1. The undersigned, being a corporation existing under the laws of the state of 74 , a

been familiarized with the specifications contained herein, hereby proposes to furnish all
supervision, personnel, labor, materials, machinery, tools, appurtenances, equipment and all
services required to design,produce, and mail the Spring 2014 Storm Water Education Brochure,
all in accordance with the specifications and at the bid prices for the following items:

Furnish mock-ups for City's approval from materials and information

2 Print, fold and tape 35,000 brochures on 70 lb. gloss paper. ;OS
Deliver the brochures to the post office for distribution under the
3 producer's bulk rate postage. Postage is not included with this
proposal as it will be determined later and will be invoiced to the
City.
