To Whom It May Concern:
AMI Environmental (AMI) is pleased to present this proposal to survey the former Rath
Administration facility for asbestos-containing materials (ACMs), lead-containing materials
(LCMs) and miscellaneous hazardous building materials (MHBMs). AMI's proposed scope of
work and associated fees are as follows:
UNDERSTANDING OF SCOPE
The area(s) to be surveyed shall include all accessible common areas, corridors, closets, offices,
rooms, attics, mechanical spaces, pipe tunnels and ceiling plenums, as well as all accessible
exterior portions of the structure.
ASBESTOS SURVEY
AMI will visually inspect the proposed survey area to determine the presence of ACMs. Pursuant
to this inspection, AMI will:
• Collect bulk samples of all identified materials suspected of being ACMs. All
asbestos bulk samples shall be analyzed by a National Voluntary Laboratory
Accreditation Program (NVLAP)-certified laboratory in accordance with the
Environmental Protection Agency (EPA) recognized method, Polarized Light
Microscopy(PLM).
Note: Certain materials that test positive for asbestos in sufficiently low
concentrations may warrant point count analysis. Point counting is a more
accurate, and more expensive, analytical method that AMI recommends when
there is reasonable likelihood that certain materials may prove to be non-asbestos-
containing. AMI will make such recommendations, as appropriate, prior to

submittal of the final survey report. Costs for performing point count analysis are
not included in this proposal.
• Identify and quantify all materials confirmed to be ACM, by type and location
within the survey area.
• Take representative photographs of all suspected ACMs; and photographs of site
conditions, as may be warranted.
OPTIONAL-LEAD XRF SURVEY
LCMs, including building fixtures, components and paint, may be present in the proposed survey
area. Subject to previous remodeling activity, the most common occurrences of lead in facilities
of this age and type include surfaces and components painted with lead-based paint, ceramic tile,
porcelain fixtures, fire doors and certain specialized equipment associated with medical services.
Depending on actual renovation and construction activities to be performed, information
concerning lead and lead-based paint may be needed by the general contractor for employee
protection purposes and proper disposal of demolition waste and debris.
AMI will inspect the proposed survey area to determine the presence of LCMs, utilizing a portable
XRF instrument. Pursuant to this inspection,AMI will:
• Collect a sufficient number of XRF readings within the work area, including each
component and/or surface with a distinct painting history, to determine the
presence of lead.
• Take representative photographs of all lead-containing components and surfaces;
and photographs of site conditions, as may be warranted.
OPTIONAL-MISCELLANEOUS HAZARDOUS BUILDING MATERIALS SURVEY
Miscellaneous hazardous building materials (MHBM) requiring special handling, disposal or
recycling may be present in the proposed survey area. Subject to previous remodeling and/or
maintenance activities, the most common occurrences of MHBMs for facilities of this age and
type include mercury within fluorescent light tubes, switches and thermostats; PCB-containing
light ballasts; batteries within emergency back-up equipment; radioactive sources; and ozone-
depleting substances within air-conditioning and water cooling units. Depending on actual
construction activities to be performed, information concerning MHBMs may be needed for
proper disposal or recycling, and employee protection purposes.
AMI will inspect the proposed survey area for occurrences of MHBMs, which may require special
handling, disposal or recycling, due to renovation activities. Pursuant to this inspection,AMI will:
• Visually identify and quantify occurrences of MHBMs in the proposed survey
area, by type and location within the survey area.
• Take representative photographs of identified MHBMs; and photographs of site
conditions, as may be warranted.

SURVEY REPORT
AMI will produce and deliver two copies of a detailed report presenting the findings of the
survey. The report shall include:
• A narrative description of the survey performed, its findings and
recommendations, if appropriate.
• Tables summarizing all ACMs identified during the survey, as well as lead-
containing paint and miscellaneous hazardous building materials.
• Analytical results for all materials analyzed for asbestos, as well as all lead XRF
instrument readings.
• Color photos showing examples for each type of ACM identified during the
survey, as well as photos of lead-based painted surfaces and lead-containing
components, and miscellaneous hazardous materials.
• 81/2"x 11" color schematics depicting the locations of ACMs identified and their
respective sample locations, as well as lead-containing surfaces and components,
and miscellaneous hazardous materials.
• Engineering estimates to abate all ACMs, as well as estimates to dispose, recycle
or otherwise addressing issues concerning lead and MHBMs.
PROJECT SCHEDULE
Asbestos survey report shall be delivered within seven (7) days from the notice to proceed
(signed contract). Four (4) additional days will be required for the lead and miscellaneous
hazardous building materials.
REGULATORY STANDARDS
AMI will perform all inspection and analytical services in accordance with all applicable federal
and state requirements, including EPA National Emission Standard for Hazardous Air Pollutants
(NESHAP) standards for asbestos (40 CFR Part 61). Personnel conducting asbestos inspections
will be certified in accordance with EPA (40 CFR 763) and State of Iowa regulations.
PROPOSAL STIPULATIONS
The following fees are subject to the following stipulations:
• Owner to provide full access to facility at time of inspection.
• Lead inspection to be considered a screening only but detailed enough for
construction purposes.
• AMI estimates 65 additional samples to be collected. Information from previous
inspection to be utilized.
• Due to the unsafe condition of the facility, AMI requires a two person team for
this project.

SURVEY FEES
Survey fees include all labor, materials, equipment and supplies to perform the proposed services
as presented herein.
The lump sum fee to perform the Asbestos Inspection &Report: $4,900.00
(Includes $70.00 for Iowa Sales Tax on Analytical Fees)
OPTIONAL
The lump sum fee to perform the Lead&Misc Haz. Mat. Assessment: $2,390.00
Total: $7,290.00
The fees in this proposal may change if the proposal is not accepted within 60 days.
