4/_7-e8' PERMIT FOR BLASTING Date April 7, 2008 THE CITY OF WATERLOO, IOWA hereby grants Chicago Explosive Services 141 W. Jackson Suite 2007 Chicago, IL 60604 permission to possess, store and use explosive materials (explosives, blasting agents, water gels, and detonators) for the purpose of Demolition of the John Deere Waterloo Works abandoned powerhouse The storage and use of explosives shall be in accordance with the applicable requirements of pamphlet number 495 of the National Fire Protection Association. The specific conditions of this permit shall be as follows: 1. The permittee may purchase an amount of explosive materials totaling no more than 27.5 pounds for the entire job. 2. The maximum amount of explosive materials in the permittee's possession on any one day shall be limited to the anticipated amount to be used during that day, or 27.5 pounds. 3. Detonations shall be limited to the immediate area of the construction project which is generally described as: John Deere Waterloo Works abandoned powerhouse 4. Blasting shall only be done between the hours of 11:00 A.M. and 4:00 P.M., Saturday or Sunday. 5. General storage of explosive materials for over-night storage will not be utilized with this project. 6. Daily storage of explosive materials not in actual use shall be in an approved magazine at the detonation location. The permittee shall be solely responsible and liable for and shall fully protect and indemnify the City against all claims for damages to person or property occasioned by or resulting from blasting or other methods or processes in the work of construction, whether such damages be attributable to the negligence of the contractor or his employees, or otherwise. This is a five story structure supported b,, 12 steel H beam columns. CBS will explosively cut and kick out eight columns allo '.._ the structure to tip to the eat. CES will use 25 lbs of LSC to sever the beams. We will shoot the front four columns i three places and the centers in two places to allow the structure to pivot off the rear columns and tip into the drop zone. All charges will be shot without delay to assure all the columns clear allowing the structure to tip_ Patrick Carney will be the blaster in charge.A copy of his resume has been given to Jo Deere_ Blast Protection-Matting We will cover the exposed linear shaped charges with conveyor belt matting to reduce copper throw and hold down air blast Initiating System CES will use a non-electric delay system to eliminate problems encountered wi static, induced or stray currents or the use of radios. Seismic monitoring CES will utilize GPS-45 seismographs to monitor vibration and air blast One seismograph will be placed directly across from the structure to be shot and another at a e occupied building Notifications 'ON y,,. Traffic Control CES/Anderson world block site traffic and access at the time of the blast.This will be coordinated with John Deere security. Blast Communications and Protection CES/Anderson would furnish radios the morning of the shot to involve. parties to assure site protection and traffic control. We would'use hlact sirens(Banshee 120)sounded at agreed upon times prior to the shot and after the shot designating an"- Clear".We have included a copy of our standard safety program with this proposal.( :e attachment A). Shot loading and Guarding Anderson would like to shoot Saturday morning to least interfere with traffic and par ',g. We would bring in the charges Saturday AM and then we will load the shot and shoot between 12:00PM and 2:00 PM. Misfire Prevention Program Misfires on the John Deere project could occur for three possible reasons: 1)Malfunction of the charges 2)Malfunction of the p-3 boosters 3)Malfunction of the initiating blasting cap, lead line or blasting machine To avoid any possibility of misfires,all charges have been randomly tested on plates they will be severing on the bridge.All charges have been ordered to sever 1/4 inch in excess of the steel on the bridge.All p-3 boosters are randomly tested before us• and the blasting machine is tested prior to its use on the day of the blast.The use of n• •1 caps and primacord as an initiating system allows CES to visually inspect the detona 'g system to ensure that the caps and lead line are operable.
