ADDENDUM TO CONTRACT

PROGRAM

This addendum to Contract is mutually agreed and entered into this 7 Day of

Both parties hereby mutually agree to extend the term of contract for a period of(2) years
from date of this approval and to implement the terms and schedule of compensation as
per the Contractor Letter of Proposal hereby attached.
