The City of Cedar Falls, City of Waterloo, and Black Hawk County, herby accepts the office supply quote,
submitted by Matt Parrott& Sons Company of Waterloo, Iowa, upon the following terms and conditions:
1. Matt Parrott& Sons shall provide office supplies to each department for the City of Cedar Falls,
City of Waterloo, and Black Hawk County at 10%above cost for the period of October 1,2008—
June 30,2009. For the second year of this contract, July 1,2009—June 30,2010, prices will not
increase more than 2%over the cost quoted for the period ending June 30,2009. There will be no
delivery charges.
2. Matt Parrott& Sons shall provide each individual department of the three governmental agencies
separate billings for office supplies, as well as new sale catalogs every six months.
3. The prices designated in the bid received from Matt Parrott& Sons shall be the prices charged to
the three governmental agencies for all office supply items supplied to it for the period October 1,
2008 through June 30,2009. (Per the attached spreadsheet)
4. This agreement incorporates by reference the RFP as a part of the contract.
5. The City of Cedar Falls, City of Waterloo,and Black Hawk County reserves the right to cancel
this agreement at any time by giving Matt Parrott& Sons a thirty(30)day advance written notice.
6. This agreement may be extended for an additional two years if agreed upon by all parties.

AGREEMENT FOR PURCHASE OF PAPER SUPPPLY
The City of Cedar Falls, City of Waterloo, and Black Hawk County, herby accepts the Paper
Supply quote, submitted by Matt Parrott& Sons Company of Waterloo, Iowa, upon the following
terms and conditions:
1. Matt Parrott& Sons shall provide paper supply to each department for the City of Cedar
Falls,City of Waterloo, and Black Hawk County for the period of October 1 through
June 30, 2010;prices are subject to change quarterly. A 30-day notice will be given to all
three government agencies when prices increase. There will be no delivery charges.
2. Matt Parrott& Sons shall provide each individual department of the three government
agencies separate billings for paper supply, as well as new's#le catalogs every six months.
3. This agreement incorporates by reference the RFP as a part of the'sontract.
4. The City of Cedar Falls, City of Waterloo, and Black Hawk Coufry reservesskhe right to
cancel this agreement at any time by giving Matt Parrott& Sobs a thirty(30)day advance
written notice.
5. This agreement may be extended for an additional two years if agreed upon by all parties.

AGREEMENT FOR PURCHASE OF INK AND TONER CARTRIDGES
The City of Waterloo herby accepts the office supply quote submitted by Matt Parrott& Sons Company of
Waterloo,Iowa, upon the following terms and conditions:
1. Matt Parrott& Sons shall provide ink and toner cartridges to each department of the City of
Waterloo at 10%above cost for the period of October 1,2008 -June 30,2009. For the second
year of this contract,July 1,2009—June 30,2010, prices will not increase more than 2%over the
cost quoted for the period ending June 30,2009. There will be no delivery charges.
2. Matt Parrott& Sons shall provide each individual department separate billings for office supplies,
as well as new sale catalogs every six months.
3. The prices designated in the bid received from Matt Parrott& Sons shall be the prices charged to
the City of Waterloo for all ink and toner cartridges supplied to it for the period October 1,2008
through June 30,2009. (per the attached spreadsheet)
