Reason for Change Order:
2 — Quantity adjusted based on field measurements — during the installation of joint and crack sealant the length of
cracks found in the field were more than was inventoried during the design survey. Additional joint and crack
repair was more extensive on the north 1,000 feet of the project than in the design survey. In addition, many areas
for crack repairs that were previously completed showed signs of failure and were added to the construction
contract. The contractor agreed to extend the unit price cost for this additional joint and crack sealing work item.
5 — Quantity adjusted based on field measurements — Less area required asphalt surface treatment than shown in
the plans.

6 — Quantity adjusted based on field measurements — additional pavement marking removals were required at the
areas on Taxiway B and B-1 to provide pavement markings within the runway hold area with the same reflectivity
and paint coverage.
7 — Quantity adjusted based on field measurements
8 — Quantity adjusted based on field measurements — not all reflective white paint required remarking as shown in
the plans.
9 — Quantity adjusted based on field measurements — black paint was not used to the quantity identified in the
plans.
10 — Quantity adjusted based on field measurements — reflective media quantity was reduced with the reduction in
yellow and white pavement markings.
11 — Quantity added based on field measurements — Runway surface painted hold position signs were not
originally included in the contract documents. Surface painted signs were placed on Taxiway A east of Runway
12/30 and on Taxiway E.

THIS DOCUMENT SHALL BECOME AN AMENDMENT TO THE CONTRACT AND ALL
STIPULATIONS AND COVENANTS OF THE CONTRACT SHALL APPLY HERETO.
