Description of Change
3a Remove 1 1/2 inches of unitube header and cap the ADD
open end with a 1/4-inch-thick plate. These changes are
in accordance with the Cost Proposal
Request (CPR) 002 dated June 28, 2024, and Request
for Change (RFC) 8 dated July 3, 2024.
3b Provide a 1/4-inch-thick steel plate to cover the pitting ADD
and holes in the influent well (on both sides of pitting
and holes, welded all around). These changes are in
accordance with CPR 002 dated June 28, 2024, and
RFC 9R dated July 10, 2024.
3c Provide eight new floor pressure relief valves equally ADD
spaced at a radius of 32 feet 0 inches from the center of
the clarifier. These changes are in accordance with
CPR 003 dated July 10, 2024 and RFC 10 dated
July 15, 2024.
3d Extend Contract Substantial Completion and ADD
Final Completion dates by 14 calendar days in
accordance with RFC 9R dated July 10, 2024 and
RFC 10 dated July 15, 2024.

The paint preparation process exposed sections of rust holes in the Influent Well. This RFC includes providing
fabricated 1/4" plate steel as needed to cover both faces of the sections of rust holes. The plates will be welded
into place with a continual weld and painted to match the balance of the clarifier.
