Re: Consulting Services Proposal
As requested, we are submitting this proposal to provide benefit consulting services to
the City of Waterloo.
Scope of Services
1. Prepare self-funded medical, Rx and dental plan cost and funding projections for
the 2009-10 fiscal plan year. These projections will be based on the current plan
of benefits.
2. Model the cost impact of up to ten different potential benefit plan design changes
identified by the City.
Timeframe
If retained by the City by November 7, 2008, we can complete the above services by
December 19, 2008. Meeting this deadline assumes that we receive the needed claims
and cost data from the City and its claim payers in a timely fashion.
Fees
Our fees to provide the scope of services described above are:
1. Cost projections: $3500
2. Plan design change modeling: $2500 for up to ten changes. Additional changes
above ten are $250 per change.
We hope to have the opportunity to assist the City with these important projects. If our
