Dear Mayor Hurley:
Enclosed please find a copy of the fully executed JMPST Agreement Amendment with the Iowa Department of
Economic Development(IDED). Please reference the original Agreement number(09-JMPST-09) in all
correspondence relating to it.
As a reminder for requesting additional funds:
You will need to submit:
• a signed original GAX Form and two (2) copies for each payment request submitted
• a copy of the County's/City's check to sub-contractor, i.e. COG, Chamber of Commerce, etc.
• copies of loan checks to businesses
• copies of corresponding promissory notes executed with businesses
Questions concerning the completion of the GAX form should be directed to Katie Caggiano, Budget Analyst, at

Please submit the required documents to me at the contact information listed below. Please note that this GAX form
is used ONLY to request State Funds.Additional funds cannot be made available until these forms are signed and
returned to our office.
Subsequent requests can be directed to me. Please limit request for funds to no more than one(1) request per week.

AMENDMENT TO GRANT AGREEMENT 'a
IDED Grant Agreement#09-JMPST-09 (Amendment No. 1)
THIS AMENDMENT is made to a certain Grant Agreement dated the 17th day of September,2008(herein referred
to as the Agreement),by and between the City of Waterloo,Iowa and the Iowa Department of Economic Development
(referred to as"IDED"),
WITNESSETH:
WHEREAS,the City of Waterloo,Iowa and IDED have heretofore entered into a certain Grant Agreement
(Jumpstart Small Business Assistance Program Grant Agreement#09-JMPST-09)effective as of the 17th day of September,
2008;and,
WHEREAS,pursuant to said Agreement,IDED has agreed to provide funding for the Jumpstart Small Business
Assistance Program to be paid to the City of Waterloo,Iowa;and,
WHEREAS,the City of Waterloo,Iowa and IDED desire to amend the Grant Assistance Agreement dated the 17th
day of September,2008 in order to modify provisions set forth in Maximum Grant Amount and Maximum Administration Fee;
funding relating to the amounts in which the aforesaid Agreement shall allow:
NOW,THEREFORE,the parties agree to amend page 1 of the Grant Agreement dated the 17th day of September,
2008 as follows:
