Former Buildings—Asbestos Inspection

Scope of Work
AUGUST ENTERPRISES, L.L.C. proposes to inspect, bulk sample and identify
asbestos-containing materials on the buildings site in 24 2 20 1 .�,., c nt- �,o ;,f--

Consultant Responsibility: We will provide
1. Necessary labor,material, supply
2. Insurances,report of laboratory findings (presence, location, quantity)
Owner Responsibility:
1. Authorize us to proceed in your behalf
2. Provide access, coordination, and liaison with trades
3. Inform us of "conflict of interest" between inspection and removal bidding
Duration:
1. It is intended that this project will be started after award and agreed upon timeline
2. The Owner will advise if any change in schedule is necessary
Exclusions:
1. This quotation is good for thirty days
2. Invoices unpaid after thirty days are past due and subject to a monthly finance
charge of five percent
3. For listing as "additional insured"there will be a fee of$500.00
4. For bonding there will be a fee of$950.00 and 5%.
