Dear Joel,
Based upon the information you provided from the main drain questionnaires and pictures, ACCO
offers the following options for your main pool and slide suction outlet at the Gates Pool, Pricing for the
plunge pool covers will be sent after we receive additional information and pictures,
Installation of 6 ASME/ANSI A 112.19.8-2207 certified covers with 2 anti vortex reducing devices at the
main pool and 2 ASME/ANSI A 112.19.8-2207 stainless steel covers at the slide suction outlet will cost
$9,364.00. This price is based upon the main pool and slide being drained, CLEAN and dry prior to
ACCO's arrival. The anti vortex devices are necessary because the measurement from the top of the
sump pipe to the bottom of the grate is 4" and it should be 12" to be in compliance.
The cost for these products is $6,994.00 plus freight, should you elect to install yourself.
ACCO will schedule the above service upon receipt of the acceptance and confirmation section of this
letter. Please sign and return to ACCO via fax at (515) 278-2183 or mail your election. Please note
that all services are being scheduled based upon the order the confirmations are received and product
availability.
ONCE ACCO HAS RECEIVED YOUR CONFIRMATION, ACCO WILL PROVIDE NOTICE TO THE
STATE OF IOWA ON YOUR BEHALF OF THE INTENDED ACTIONS.

which is an annual percentage rate of 18%. Should ACCO be required to initiate legal proceedings to collect any balance due under this
proposal, Customer agrees to pay reasonable attorney's fees, cost and Incidental expenses incurred in collection
