We hereby propose to furnish all the materials, equipment and perform all labor for the completion of:
Asbestos removal at the above referenced project:
Removal based on inspection report:
Proposal Includes: 1) Personal and excursion air sampling, 2)All Disposal 3) Removal
10 day notification to the Iowa DNR and Iowa OSHA
Owner's responsibilities:
WE PROPOSE to furnish material and labor, complete in accordance with the above specifications for the
sum of: Eleven Thousand Nine Hundred Dollars ($1 1 ,900.00*)
Terms: Net 30 Days! All accounts subject to 1.5% monthly charge after 30 days.
All work is guaranteed to be as specified. All work to be completed in a workmanlike manner Respectfully Submitted,
according to specified practices. Any alteration or deviation from the above specifications
involving extra costs will be executed only upon written orders and will become an extra charge
above the estimate. All agreements contingent upon strikes,accidents or delays beyond our
control. Owner to carry fire,tornado,and other necessary insurance. We carry general liability ADVANCED ENVIRONMENTAL
insurance and our workers are fully covered by workers compensation insurance. TESTING AND A 'TEMENT, INC.
NOTE: This proposal maybe withdrawn p p by us if not
accepted within 45 days. ��
