Dear Mr. Temeyer:
Terracon appreciates the opportunity to submit this proposal to conduct a Limited Site
Investigation at the above referenced site.
A. PROJECT INFORMATION
The purpose of this proposal is to conduct a Limited Site Investigation (LSI) at the above-
referenced property (hereinafter, the site). The proposed scope of work is based on the results
of Terracon's Phase I Environmental Site Assessment (ESA — Report No. 13027726) dated
April 1, 2002 that identified potential environmental concerns associated with current and
historical activities at various properties within the Base Area.
The objective of the proposed LSI is to provide additional information regarding the potential
presence of elevated chemicals of concern (COC's) in the on-site soils and/or groundwater as a
result of a potential release from these various properties. Since many REC's are located where
buildings/businesses currently exist, our rationale for such sites is to investigate in the presumed
downgradient position from the REC. Where possible, REC's and wells placed to investigate these
REC's were combined to optimize resources at the request of the City of Waterloo. Where COC's
are detected above statewide standards, additional investigation may be warranted.
B. SCOPE OF SERVICES
1. Base Services
Following receipt of a signed Notice to Proceed and no later than 48 hours prior to intrusive
activities, Terracon will contact One-Call to arrange for underground utility locates at the above
referenced site.
Terracon will develop a generic safety plan to be used by our personnel during field services.
At this time, we anticipate that a modified USEPA Level D work uniform consisting of hard hats,
safety glasses, latex gloves, and steel-toed boots will be required by all personnel in the work

area. It may become necessary to upgrade this level of protection, at additional cost, while
sampling activities are being conducted if petroleum or chemically saturated soils/liquids are
encountered and there is increased risk for personal exposure.
A total of three (3) temporary monitoring wells have been requested by the City of Waterloo at
following locations:
TMW-1 Site 85 — 300 West Park Avenue Former gas station
TMW-2 Site 85 — 300 West Park Avenue Former gas station
TMW-3 Site 84 — 310 West Park Avenue Indications of UST
The temporary monitoring wells will be placed to a depth of approximately 20 feet below ground
surface (bgs), or five feet below initial water table or auger refusal, whichever occurs first. The
proposed total depths will not be exceeded without verbal approval from the City of Waterloo.
Additional drilling required beyond these depths will be charged at a rate of$30.00 per foot.
Drilling equipment will be decontaminated by high-pressure cleaning prior to commencement of
the project and between the installation of each boring. Non-dedicated sampling equipment will
be decontaminated using an Alconox° detergent wash and potable water rinse prior to
commencement of the project and between collection of each sample.
Soil samples will be collected continuously using core barrels or split spoon samplers in all wells
located on the suspect REC sites to document lithology, color, relative moisture content and
visual or olfactory evidence of VOCs. Soil samples for borings/wells placed off-site will be
sampled at 5-foot intervals. The samples will be field screened with a photoionization detector
(PID)for the presence of VOCs in soil.
The temporary monitoring wells will be completed as follows:
• Installation of 10 feet of 2-inch diameter, 0.010-inch machine slotted PVC well screen with a
threaded bottom cap;
• Installation of 2-inch diameter, threaded, flush-joint PVC riser pipe to surface.
Following development of the monitoring wells and prior to groundwater sample collection, each
well will be purged. Purging will be performed by removing groundwater with a new, disposable,
bailer until the monitoring well formation fails to recharge (i.e., well runs dry); or a minimum of
three well casing volumes of groundwater is removed, whichever occurs first.
Following completion of sampling activities, the temporary monitoring wells will be properly
abandoned in general accordance with applicable state regulations and guidelines. The
temporary monitoring wells will be backfilled with bentonite pellets and soil cuttings. An asphalt
or concrete patch will be placed at the surface, where applicable. The client should understand
that some settlement of the borehole fill may occur and no future maintenance or filling of the
holes is included in our fee. If the client is concerned about borehole settlement, alternative

backfill methods such as sand or bentonite grout could be performed. If requested, these
services would be charged in accordance with our standard unit rates and hourly charges.
Sampling Program
Terracon's soil and groundwater sampling program is outlined in the Table below:
For borings/wells installed on-site and investigated for VOC's, one soil sample from each soil
boring from the zone exhibiting an elevated PID readings will be collected. If no evidence of
elevated PID is noted, no soil sample will be collected.
One groundwater sample from each groundwater monitoring well using a new, disposable bailer
will be collected. One sample will be collected as a quality assurance / quality control (QA/QC)
field blank to be analyzed for VOCs to verify sample integrity during collection and transit.
The soil and groundwater samples will be collected in laboratory prepared containers, labeled,
and placed on ice in a cooler that will be secured with a custody seal. The samples and
completed chain-of-custody forms will be transported to the selected analytical laboratory for
standard turnaround.

A summary of the analyses, sample type and USEPA-approved methods are presented below:

Preparation of LSI Final Report
Upon completion of site activities, a final report will be prepared that will include the following:
• Documentation of field activities;
• Site plan showing pertinent site features;
• Soil boring/monitoring well logs;
• Analytical laboratory results;
• Data evaluation and presentation of pertinent findings; and,
• Recommendations concerning further action, if necessary.
Schedule
It is the responsibility of the client to secure access to the site to perform the investigation. Terracon
is prepared to commence work on this project within five days following receipt of written notification
to proceed. It is anticipated that the field activities will be completed in one to two days. Preliminary

verbal results of the LSI may be available within 48 hours of Terracon's receipt of laboratory
analysis. The final written LSI Reports will be available within two weeks following Terracon's
receipt of final laboratory analytical reporting. This letter report will reflect final results, findings and
recommendations, and, as such, will take precedence over any verbal reports that Terracon
personnel may have provided.
In order to comply with the proposed schedule, the following items are required to be provided by
the client at the time of notification to proceed in order to meet the client's required project
completion date. Please include the following requested items along with the notification to
proceed:
• Right of entry to conduct the field services.
• Notification of any restrictions or special requirements (such as safety) regarding
accessing the site
• A signed Agreement for Consulting Services evidencing acceptance of this scope of
services
C. RELIANCE
The LSI Report will be prepared for the exclusive use and reliance of the City of Waterloo. Use
or reliance by any other party is prohibited without the written authorization of the City of
Waterloo and Terracon.
If the client is aware of additional parties that will require reliance on the LSI Report, the names,
addresses, and relationship of these parties should be provided to Terracon for approval prior to
the time of authorization to proceed. If, in the future, the client and Terracon consent to reliance
on the LSI Report by a third party, letters of reliance will be prepared for an additional cost of
$250 per relying party.
Reliance on the LSI Report by the client and all authorized parties will be subject to the terms,
conditions, and limitations stated in the proposal, LSI Report, and Terracon's Master Services
Agreement. The limitation of liability defined in the Terms and Conditions is the aggregate limit of
Terracon's liability to the client and all relying parties.
The limitations herein must be considered when the user of this report formulates opinions as to
risks associated with the site. No warranties, express or implied, are intended or made.
D. COMPENSATION
The Scope of Services outlined in this proposal will be performed on a time and material basis.
If, as a result of these services, additional work is required outside the scope of this proposal,
you will be contacted, and upon request, proposed costs for additional work will be provided on
a time and materials basis. Client authorization will be obtained prior to commencement of any

additional work outside the scope of this proposal. Estimated project costs are summarized
below:

The project cost summary is based on the scope of services outlined in this proposal and will be
billed on a lump sum basis and assumes all field services will be performed under modified
Level-D safety, personal-protective procedures.
Transportation and disposal of removed soil cuttings and/or development water and other waste
materials are out-of-scope services and not included in this proposal. Terracon anticipates
generating approximately two to three drums of soil, groundwater, and/or washwater during the
LSI activities. If the material is hazardous, then a hazardous waste TSD will need to be
contracted to transport and dispose of the material. Terracon would be pleased to provide a
separate proposal for such services once a final volume of waste is determined.
The fee is valid for 60 days from the date of this proposal and is based on the assumption that all
field services will be performed under safety Level D personal protective procedures and that only
one site visit will be required by Terracon personnel. If these assumptions are not valid, there will
be additional charges.
E. AUTHORIZATION
This proposal may be accepted by executing both originals of the attached Master Services
Agreement and returning one copy to Terracon. Services will be initiated upon receipt of
the written notice to proceed.
The terms, conditions and limitations stated in the Agreement, and sections of this proposal
incorporated therein, shall constitute the exclusive terms and conditions and services to be
performed for this project. This proposal is valid only if authorized within sixty days from the
proposal date. Project initiation may be expedited by faxing a copy of the signed Agreement to
(319) 277-4320, provided a complete executed copy is received by Terracon within 5 days.

TASK ORDER
This TASK ORDER is issued under the MASTER SERVICES AGREEMENT(dated 2/9/05 agreement reference number
CF05042 )between City of Waterloo ("Client")and Terracon Consultants, Inc.
("Consultant")for Services to be provided by Consultant for Client on the City of Waterloo Project
("Project"),as described in the Project Information section of Consultant's Task Order Proposal dated 11/6/2006
("Task Order Proposal")unless the Project is otherwise described below or in Exhibit A to this Task Order(which section or Exhibit are incorporated
into this Task Order). This Task Order is incorporated into and part of the Master Services Agreement.

B. Scope of Services The scope of Services to be provided under this Task Order are described in the Scope of Services section of the
