TO: K. Cunningham Construction Co., Inc. , Contractor
You are hereby ordered to make the following changes from the plans and specifications or perform
the following extra work on your contract dated July 20, 2009.
A. Description of change to be made or extra work to be done:
1. Exempt Bid Item No. 9, Special Backfill, Subgrade Repair, from consideration under the
provisions of Section B. "Scope of Work," Paragraph 7 of the "General Specifications for
Construction", for work that varies by more than twenty (20%) percent from the estimated
quantities specified in the contract.
2. Increase Bid Item No. 9, from 100 CY to 857 CY equals a net increase of 757 CY.
Quantity for subgrade repair is based on 12-inches of depth.
3. Furnish and install geogrid (Tensar BX1100 (BR-1), TENSAR BX4100 (SG-1) or
Engineer-approved equal). 2,569 SY
B. Reason for ordering change or extra work:
Increase in subgrade repair and addition of geogrid was necessitated due to condition
of poor subgrade soils.
C. Settlement for cost of work to be made as follows:
1. No change in contract unit price of $25.25 per CY.
2. Agreement with contractor at $2.95 per SY.
NET INCREASE $26,692.80
