RESOLUTION NO. 2024-633
A RESOLUTION APPROVING THE PROVIDER
PARTICIPATION AGREEMENT FOR FY 2026, AND
AUTHORIZING THE MEDICAL SUPERVISOR, JASON
HERNANDEZ, TO EXECUTE SAID DOCUMENT.
WHEREAS, the City of Waterloo is committed to maintaining high standards of
healthcare services and participation in relevant provider networks to enhance the quality and
breadth of medical services available to its residents; and
WHEREAS, the Provider Participation Agreement for FY 2026 allows for participation
in the Ground Emergency Medical Transportation (GEMT) Uncompensated Cost
Reimbursement Program; and
WHEREAS, the Provider Participation Agreement for FY 2026 outlines the terms,
conditions, and expectations for service delivery, funding, and performance within the
designated provider framework; and
WHEREAS, approval and execution of this agreement are recommended to ensure
continuity and expansion of healthcare services, facilitated by appropriate oversight and
accountability measures.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Waterloo as
follows:
1. The Provider Participation Agreement for FY 2026 is hereby approved, delineating
responsibilities and guiding collaboration between the City of Waterloo and healthcare providers.
2. Waterloo Fire Rescue is tasked with monitoring the implementation of the agreement
effectively, ensuring compliance with agreed standards and optimizing benefits for the city's
population.
3. Jason Hernandez, the Medical Supervisor, is authorized and directed to execute the
aforementioned agreement on behalf of the City of Waterloo, formalizing his role in ensuring
compliance and service excellence.
4. This resolution shall take effect immediately upon approval, enabling timely and
effective implementation, as specified in the Provider Participation Agreement.
