You are hereby ordered to make the following changes from the plans and specifications or perform
the following extra work on your contract dated 03/18/2024.
A. Description of changes to be made or extra work to be done:
a. Contractor to supply flaggers required for work in Railroad ROW.
b. Contractor to be paid for finishing project before required completion date.
c. Contractor to be deducted for area of paving outside of smoothness specification.
B. Reason for ordering change or extra work:
a. Flaggers to be required when working in Railroad ROW.
b. Incentive paid for contractor finishing project before required completion date.
c. Deduction for area of paving outside of smoothness specification.
C. Settlement for the cost of work to be made as follows:
a. 5001- RAIROAD FLAGGING - LS/$9,475 -1 @ $9,475 = $9,475
b. 5002 - INCENTIVE - LS/$50,000 -1 @ $50,000 = $50,000
c. 5003 - PAVEMENT SMOOTHNESS DEDUCTION - LS/-$375 -1 @ -$375 = - $375
