RESOLUTION NO. 2024-717
A RESOLUTION APPROVING COMPLETION OF PROJECT
AND RECOMMENDATION OF ACCEPTANCE OF WORK
FOR WORK PERFORMED BY MIDWEST SURFACE PREP,
INC., OF NASHUA, IOWA IN THE AMOUNT OF $23,680.00,
IN CONJUNCTION WITH THE FY-23 IDOT CSVI PROJECT,
CONTRACT NO. 4606.
WHEREAS, the City of Waterloo contracted Midwest Surface Prep, Inc., of Nashua,
Iowa, to perform specified work under Contract No. 4606, in conjunction with the FY-23 IDOT
CSVI Project; and
WHEREAS, Midwest Surface Prep, Inc. has completed the project in accordance with
the contract terms and specifications to the satisfaction of the City and project engineers; and
WHEREAS the total amount for the work performed is $23,680.00, and it is now
appropriate to approve the completion and accept the work as fully performed.
NOW, THEREFORE, BE IT RESOLVED by the City Council of the City of Waterloo as
follows:
1. The City Council hereby acknowledges and approves the completion of the work
performed by Midwest Surface Prep, Inc. under the terms of Contract No. 4606 for the FY-23
IDOT CSVI Project.
2. The work completed by Midwest Surface Prep, Inc. in the amount of $23,680.00 is
hereby accepted as satisfactory and in compliance with the project specifications.
3. The City is authorized to process and issue the final payment to Midwest Surface Prep,
Inc. in accordance with the terms of the contract, acknowledging the project's completion and
acceptance.
4. This resolution shall take effect upon its passage and approval.
