Attached is Change Order No. 6 for the Titus Lift Station and Force Main project submitted by WRH, Inc. The following are
the Change Order Requests (CAR) submitted by WRH, Inc.
CAR 15 is to extend the final completion date to 05/30/2025 due to unforeseen delays. The delay is time for MidAmerican
Energy to move an existing gas main that was in the way to connect to the existing sanitary sewer. The location of the gas
main was not correct in the plans and was unknown till August. It took 4 months for MidAmerican Energy to disconnect that
gas main. Work is on schedule to have the lift station operational by 01/31/2025. The remaining work is cosmetic.
CAR 16 is a deduction for reduced demolition of the existing lift station. WRH will complete the task as previously described
except the existing wet will be abandoned in place below 10 feet instead of full removal. A full description is provided on the
attached CAR 16 from WRH, Inc. Pay Item No. 4
CAR 17 is an addition to extend the existing 6-IN force main to provide additional overflow relief. Pay Item No. 52
CAR 18 is an addition to provide additional bollards to protect the building and generator pad. Pay Item No. 53
Car 19 is an addition to increase the size of the magnetic meter from 10-IN to 12-IN. Pay Item No. 54
AECOM and City Staff recommends the City approves Change Order No. 6 for the Final Completion Date of 05/30/2025.
and for the addition of $10,527.01. The revised total contract cost is $6,294,296.30. The increase is 1.78% of the original
contract including all change orders.

Reason for Change Order
Change Order Request (CAR) 15 is to extend the final completion date to 05/30/2025 due to unforseen delays. The delay is time for

be determined based on field conditions) of which will be removed and legally disposed. The
