(Terracon) appreciates the opportunity to submit this proposal to provide a Construction Review Services (CSR) for the above referenced project as requested by the City of Waterloo. A. PROJECT INFORMATION The City of Waterloo, Iowa (City) prepared Plans, Specifications and Form of Contract for the above referenced project with sealed proposal due to the City on February 13, 2025. The extent of work involved is the demolition and plugging of the existing north relief well at the Hawthorne Avenue storm sewer lift station and the installation of a new replacement north relief well. We understand that the City has selected Cahoy Pump Service, Inc. (Cahoy) as the Contractor for the project. Terracon's subsurface exploration and engineering recommendations were utilized for the support of the design of a new relief well to replace the existing North Relief well at the Hawthorne Avenue Pumping Station site. Terracon also prepared a Drilling Program Plan (DPP) dated May 20, 2022 for the new relief well that details the well installation documentation and testing required. This plan was approved by the United States Army Corp of Engineers (USACE) on June 1, 2022. The City of Waterloo has requested that Cahoy work with Terracon to submit a completed DPP to the USACE for their approval before beginning construction. SCOPE OF SERVICES Based upon these understandings, Terracon proposes the following scope of services. Task 1 - Drilling Program Plan Terracon will meet with Cahoy to review the previously approved DPP and incorporate updated information provided by Cahoy into a revised DPP for the project for USACE approval. Task 2 - Project Support Terracon will provide construction review services as noted in the DPP to include the following: ■ Terracon will provide an Iowa DNR Certified Well Contractor to observe Cahoy's installation and development of the north replacement relief well along with the abandonment of the existing north relief well. Note, this does not replace Cahoy's Iowa DNR Certified Well Contractor responsibilities. ■ Terracon will provide an on -site field engineer during the relief well installation. The relief well boring shall be logged from cuttings by the on -site field engineer. During the borehole drilling, disturbed samples from drilling cuttings will be obtained to confirm physical soil characteristics. In the event the subsurface conditions vary from those anticipated, drilling and sampling will proceed as planned until a termination depth has been decided upon by the Geotechnical Engineer and the on -site Engineer or Geologist. The on -site Engineer/Geologist will add notes to the as -built drawings and will forward the drawings and field log to the Geotechnical Engineer for review. Prior to installing the well, hydrofracture calculations will be updated by the Geotechnical Engineer, if necessary, and the Geotechnical Engineer will communicate the revised installation procedure to the on -site Engineer/Geologist and the Contractor. ■ Terracon personnel will also be on -site to document/observe relief well development activities and relief well capacity testing. Upon project completion, Terracon will submit all relief well installation and development documentation, well diagrams with subsurface soil data and layering, and results of pumping tests to the City of Waterloo to be submitted to the USACE. Construction material testing (ie. compaction testing) is not included in this scope of service. If the City of Waterloo requests these services a Change Order will be provided. Site Access and Safety Terracon will be responsible for supervision and site safety measures for its own employees and will not be responsible for the supervision or health and safety precautions for any third parties, including contractors, subcontractors, or other parties present at the site. As such, and in accordance with our Incident and Injury Free® safety culture, Terracon will develop a safety plan for use by our personnel during field services. Prior to commencement of on -site activities, Terracon will review health and safety needs for this specific project. At this time, we anticipate performing fieldwork using personal protective equipment consisting of high -visibility clothing, hard hats, safety glasses, protective gloves, and steel -toed boots. C. COMPENSATION The scope of services as described in this proposal will be invoiced on a time and materials basis per the attached fee schedule. Task 1 Preparation of DPP Estimated fee to assist Cahoy with a revised DPP: $6,000 to $7,000 Task 2 Project Support The scope of services for Task 2 as described in this proposal will be invoiced on a time and materials basis per the attached fee schedule. For budget planning purposes, Terracon anticipates the following ■ For estimating proposes - Terracon anticipates up to seven days in the field for installation of the north replacement well and abandonment of the north existing well. This fee estimates assumes a field engineer and licensed well contractor will be onsite approximately ten hours per day and includes labor, travel expenses (mileage) and equipment charges. Estimate fee: $23,000 to $25,000. ■ Estimated fee for reporting and project management: $7,500 to $9,000 Terracon's invoice will be submitted at the completion of our services or on a 30-day billing cycle, whichever comes first. If our scope of services extends beyond 30 days, you will be invoiced for a portion of the project services that have been completed to date. Should conditions be encountered that require additional services beyond the scope of services identified in this proposal, we will contact you prior to initiating such work, explain the situation, and negotiate additional fees beyond those proposed herein. If this proposal meets with your approval, work may be initiated by returning a fully executed copy of the attached Supplement to Agreement for Services attached to this proposal to our Cedar Falls office. The terms, conditions, and limitations stated in the Agreement for Services and sections of this proposal incorporated therein, shall constitute the exclusive terms and conditions and services to be performed for this project. ("Consultant") for additional or changed Services to be provided by Consultant for Client on the FY 2024 Hawthorne Avenue Storm Sewer Lift Station Relief Well Project, as described in Flood Control Relief Well Inspection and Testing. This Change Order is incorporated into and made part of the Agreement. . Scope of Services. The scope of the additional or changed Services is described in the Scope of Services section of Consultant's Change Order Proposal, unless Services are otherwise described below or in Exhibit B to this Change Order (which section or exhibit are incorporated into the Change Order). . Compensation.
