RESOLUTION NO. 2016-423
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
DELTA DENTAL OF IOWA TO ADMINISTER THE EMPLOYEE
DENTAL HEALTH BENEFIT PLAN, INCLUDING
ADMINISTRATIVE AND CLAIMS SERVICES, AT A COST OF $4.17
PER EMPLOYEE PER MONTH FOR FY 17.
BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated June 6, 2016, for the renewal of dental health benefit plan with Delta
Dental of Iowa, to administer the employee dental health benefit plan, including administrative
and claims services, at a cost of$4.17 per employee per month, for the period of July 1, 2016
through June 30, 2017, is hereby approved and the Mayor is authorized and directed to execute
the same on behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 6th day of June, 2016.
