RESOLUTION NO. 2025-583
RESOLUTION APPROVING THE PROVIDER
PARTICIPATION AGREEMENT FOR FY 2027, AND
AUTHORIZING THE MEDICAL SUPERVISOR, JASON
HERNANDEZ, TO EXECUTE SAID AGREEMENT.
WHEREAS, the City of Waterloo recognizes the importance of participating in the Iowa
Department of Health and Human Services Ground Emergency Medical Transportation
uncompensated cost Prospective Payment Program, Provider Participation Agreement for FY
2027 to ensure consistent and quality healthcare services are accessible to its community; and
WHEREAS, the agreement outlines the terms and conditions for participation in the
program, promoting the welfare of residents through enhanced healthcare service delivery; and
WHEREAS, Jason Hernandez, the Medical Supervisor, is designated to execute the
agreement, ensuring the City's compliance with all outlined requirements and obligations; and
WHEREAS, the City Council has reviewed the provisions of the agreement and finds it in
alignment with the City's goals and healthcare objectives.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF
WATERLOO, IOWA,
1. The Provider Participation Agreement for FY 2027 is hereby approved.
2. Jason Hernandez, Medical Supervisor, is authorized and directed to execute said
document on behalf of the City of Waterloo, Iowa.
