STATUS CODE DESCRIPTION
FAIL 102.4 - Application of building The design and construction of new structures shall comply with the
code. International Building Code , and any alterations, additions, changes in use or
changes in structures required by this code, which are within the scope of the
International Building Code , shall be made in accordance therewith.
Location: All new fire door hardware installed and additional hardware for guest
rooms
Original Comment: Obtain permit and inspections for all work planned or
completed with fire doors.
FAIL 102.4 - Application of building The design and construction of new structures shall comply with the
code. International Building Code , and any alterations, additions, changes in use or
changes in structures required by this code, which are within the scope of the
International Building Code , shall be made in accordance therewith.
Location: All new fire door hardware.
Original Comment: Provide documentation to Waterloo Building Department for
all fire door hardware when applying for permit: 1010.2.9.3 Installation. Where
panic or fire exit hardware is installed, it shall comply with the following: 1. Panic
hardware shall be listed in accordance with UL 305. 2. Fire exit hardware shall be
listed in accordance with UL 10C and UL 305.3. The actuating portion of the
releasing device shall extend not less than one-half of the door leaf width. 4. The
maximum unlatching force shall not exceed 15 pounds (67 N).

FAIL 1103.9 - Carbon monoxide Carbon monoxide alarms detection shall be installed in existing dwelling units
alarmsdetection. and sleeping units Group 1-1, 1-2, 1-4 and R occupancies and in classrooms in
Group E occupancies where those units include any of the conditions identified in
Sections 915.1.2 through 915.1.6. The carbon monoxide alarms shall be installed
in the locations specified in Section 915.2 and the installation shall be in
accordance with Section 915.4. Exceptions: 1.Carbon monoxide alarms are
permitted to be solely battery operated where the code that was in effect at the
time of construction did not require carbon monoxide detectors to be provided.
2.Carbon monoxide alarms are permitted to be solely battery operated in
dwelling units that are not served from a commercial power source. 3.A carbon
monoxide detection system in accordance with Section 915.5 shall be an
acceptable alternative to carbon monoxide alarms.

rated over 1,000 volts. The minimum required working space shall be not less
than 30 inches (762 mm) in width, 36 inches (914 mm) in depth and 78 inches
(1981 mm) in height shall be providedin front of electrical service equipment.
Where the electrical service equipment is wider than 30 inches (762 mm), the
minimum working space shall be not less than the width of the equipment.
Storage of materials shall not be located within the designated working space.
Exceptions: 1. Where other dimensions are required or allowed by NFPA 70.2.
Access openings into attics or under -floor areas that provide a minimum clear
opening of 22 inches (559 mm) by 30 inches (762 mm).

and shall be listed and labeled in accordance with UL 817. Extension cords and
flexible cordsshall not be affixed to structures, extended through walls, ceilings
or floors, or under doors or floor coverings, nor shall such cords be subject to
environmental damage or physical impact. Extension cords shall be used only
with portable appliances. Extension cords marked for indoor use shall not be
used outdoors.

FAIL 610.1.2 - Maintenance. The lint trap, mechanical and heating components, and the exhaust duct system
of a clothes dryer shall be maintained in accordance with the manufacturer's
operating instructions to prevent the accumulation of lint or debris that prevents
the exhaust of air and products of combustion.

Original Comment: provide documentation of dryer vent cleaning to the outside
of the building for guest dryers and housekeeping commercial dryers.
FAIL 704.1 - Maintaining Where required when the building was originally constructed, materials and
protection. systems used to protect joints and voids in the following locations shall be
maintained. The materials and systems shall be securely attached to or bonded
to the adjacent construction, without openings visible through the construction.
1.joints in or between fire -resistance -rated walls, floors or floor/ceiling
assemblies and roof or roof/ceiling assemblies. 2.Joints in smoke barriers. 3.Voids
at the intersection of a horizontal floor assembly and an exterior curtain wall.
4.Voids at the intersection of a horizontal smoke barrier and an exterior curtain
wall. 5.Voids at the intersection of a nonfire-resistance-rated floor assembly and
an exterior curtain wall. 6.Voids at the intersection of a vertical fire barrier and an
exterior curtain wall. 7.Voids at the intersection of a vertical fire barrier and a
nonfire-resistance-rated roof assembly. Unprotected joints and voids do not
need to be protected where such joints and voids were not required to be
protected when the building was originally constructed. Where the system design
number is known, the system shall be inspected to the listing criteria and
manufacturer's installation instructions.
