following extra work on your contract dated Emergency Repair.
A. Description of change to be made or extra work to be done:
1001 Install new SW-301, 48" Sanitary Manhole.
1002 Excavate existing subgrade.
1003 Furnish and place modified subbase.
B. Reason for ordering change or extra work:
1001 Replace existing failing manhole.
1002 Remove existing unsuitable subgrade.
1003 Place and compact new subbase material for new road base.
C. Settlement for cost of work to be made as follows:
1001 SW-301, 48" - $13,560 EACH x 1 EACH
1002 Excavation - S80 /CY x 49.5 CY
1003 Modified Subbase - $25 / SY x 220 SY
