RESOLUTION NO. 2013-547
RESOLUTION APPROVING RENEWAL OF CONTRACT WITH
DELTA DENTAL OF IOWA.
BE IT RESOLVED BY THE COUNCIL OF THE CITY OF WATERLOO, IOWA,
that the contract dated June 24, 2013, to provide administrative
and claims services for the employee dental benefit plan, at a
cost of $3.80 per employee per month, for the period of July 1,
2013 through June 30, 2014, by and between Delta Dental of Iowa
and the City of Waterloo, Iowa, be and the same is hereby
approved, and the City Clerk authorized to execute the same in
behalf of the City of Waterloo, Iowa.
PASSED AND ADOPTED this 24th day of June, 2013.
