RESOLUTION NO. 2026-327
RESOLUTION APPROVING A PROFESSIONAL SERVICES
AGREEMENT WITH EMPLOYEE AND FAMILY
RESOURCES OF DES MOINES, IOWA, IN THE AMOUNT OF
$13,389.75, TO CONTINUE TO PROVIDE AN EMPLOYEE
ASSISTANCE PROGRAM, AND AUTHORIZING THE
MAYOR TO EXECUTE SAID DOCUMENT.
WHEREAS the City of Waterloo, Iowa, has determined it is in the best interest of the city
to enter into a Professional Services Agreement with Employee and Family Resources of Des
Moines, Iowa, to continue to provide an Employee Assistance Program; and
WHEREAS, the cost of said Professional Services Agreement is in the amount of
$13,389.75; and
WHEREAS, the City Council finds it appropriate to approve said Agreement and
authorize execution of the document.
NOW, THEREFORE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF
WATERLOO, IOWA,
1. The Professional Services Agreement with Employee and Family Resources of Des
Moines, Iowa, in the amount of $13,389.75, to continue to provide an Employee Assistance
Program is hereby approved.
2. The Mayor is authorized and directed to execute said document on behalf of the City
of Waterloo, Iowa.
