You are hereby notified that on January 21st the
undersigned slipped and fell on the sidewalk on Reber Ave.
fracturing two bones in his right wrist ;
That the sidewalk was covered with an accumulatign
of ice and snow, and on account of this neglected and
danz,erous condition of the sidewalk the undersigned was
injured;
That he has witnesses to the condition of the
sidewalk;
That on the day of the accident he notified the
Mayor of hi injury;
That he has a family to provide for , and by :.eason
of his injury he was unable to work for two weeks;
That if settlement is made at this time he is willing
to settle on a basis of actual cost to him, as follows :
