of the City of Waterloo , Iowa.
You are hereby notified that on January 21st the
undersigned sli,)ped and fell on the sidewalk on Reber Ave.
fracturing two bones in his right wrist ;
That the sidewalk Wall covered with an accumulatilm
of ice and snow, and on account of this neglected and
dangerous condition of the sidewalk the undersigned was
injured;
That he has witnessos to the condition of the
sidewalk;
That on the day of the accident he notified he
Mayor of his injury;

of hi injury ha was unable to work for two weeks;
That if ettlIment is made at this time he is willing
to settle on a basis of actual cost to him, ms follows:
