FAIL 603.5.3-Installation. Relocatable power tap cords shall not extend through walls,ceilings,floors,under
doors or floor coverings,or be subject to environmental or physical damage.
Inspector:JEFF SIEBEL-Comments:remove power taps from wall cavities going to
Point of sale register.Install permanent power with electrical permit and inspections.
FAIL 603.6-Extension cords. Extension cords and flexible cordsshall not be a substitute for permanent wiring and
shall be listed and labeled in accordance with UL 817.Extension cords and flexible
cordsshall not be affixed to structures,extended through walls,ceilings or floors,or
under doors or floor coverings,nor shall such cords be subject to environmental
damage or physical impact.Extension cords shall be used only with portable
appliances. Extension cords marked for indoor use shall not be used outdoors.

kitchen to bar plugged into power strip and appliances.Appliances must plug directly
into wall outlet. Install permanent power with electrical permit and inspections.

suppression inspection report with all discrepancies corrected.Current inspection
from proshield indicates deficiency.Contractor must upload corrections online to the
compliance engine.
FAIL 704.1 -Maintaining Where required when the building was originally constructed, materials and systems

materials and systems shall be securely attached to or bonded to the adjacent
construction,without openings visible through the construction. 1.joints in or
between fire-resistance-rated walls,floors or floor/ceiling assemblies and roof or
roof/ceiling assemblies.2.Joints in smoke barriers.3.Voids at the intersection of a
horizontal floor assembly and an exterior curtain wall.4.Voids at the intersection of a
horizontal smoke barrier and an exterior curtain wall.5.Voids at the intersection of a
nonfire-resistance-rated floor assembly and an exterior curtain wall.6.Voids at the
intersection of a vertical fire barrier and an exterior curtain wall.7.Voids at the
intersection of a vertical fire barrier and a nonfire-resistance-rated roof assembly.
Unprotected joints and voids do not need to be protected where such joints and
voids were not required to be protected when the building was originally
constructed.Where the system design number is known,the system shall be
inspected to the listing criteria and manufacturer's installation instructions.

FAIL 904.13.1 -Manual system A manual actuation device shall be located at or near a means of egress from the
operation. cooking area not less than 10 feet(3048 mm)and not more than 20 feet(6096 mm)
from the kitchen exhaust system.The manual actuation device shall be installed not
more than 48 inches(1200 mm)nor less than 42 inches(1067 mm)above the floor
and shall clearly identify the hazard protected.The manual actuation shall require a
maximum force of 40 pounds(178 N)and a maximum movement of 14 inches(356
mm)to actuate the fire suppression system. Exception:Automatic sprinkler systems
shall not be required to be equipped with manual actuation means.

off of wall and will not operate correctly.
FAIL 906.4.2-Class K portable fire Where hazard areas include deep fat fryers,listed Class K portable fire extinguishers
extinguishers for deep fat shall be provided as follows: 1.For up to four fryers having a maximum cooking
fryers. medium capacity of 80 pounds(36.3 kg)each:one Class K portable fire extinguisher
of a minimum 1.5-gallon(6 L)capacity.2.For every additional group of four fryers
having a maximum cooking medium capacity of 80 pounds(36.3 kg)each:one
additional Class K portable fire extinguisher of a minimum 1.5-gallon(6 L)capacity
shall be provided.3.For individual fryers exceeding 6 square feet(0.55 m2)in surface
area:Class K portable fire extinguishers shall be installed in accordance with the
extinguisher manufacturer's recommendations.
Inspector:JEFF SIEBEL-Comments:remove obstructions to k-guard fire extinguisher
in kitchen.(remove appliances etc or relocate fire extinguisher).maintain a minimum
of 36"clearance.to fire extinguisher. Fire extinguisher should be 4"to 5'above floor
in a readily accessible area.K-guard must be between 10 and 20 feet from fryer.
