GENERAL NOTES BROCK WELIVER-08/18/2022 @ 14:33 A complaint was received by FM Weliver in regards to this property. The complaint was made by property owner John Hayes. The main items of the complaint were 1)hinge kits requirements on the kitchen exhaust hood,2)requirements for grease interceptors in the plumbing, and 3)issues with Commercial Building Inspector Jeff Siebel performing inspections on his(John Hayes)properties. Item#1:this building and kitchen hood exhaust system already have a required hinge kit installed. This has been in place for for some time and is not relevant to the annual fire inspection performed on this premises. Item#2:the required grease interceptor is not in the scope of the the annual commercial fire inspection on this property.Any questions or comments in regards to this issue need to be addressed with the Building Dept., specifically the Pluming Inspector. Item#3:The business operating at this location,3 Amigos, has an alcohol license which is regulated by both local and state regulations which stipulate that an annual fire inspection is to be performed in accordance with those regulations. When the business submits their required paperwork on an annual basis, this starts the timely filing process which involves the annual fire inspection. This process is generally started at the same time every year for this business. The filing of the required paperwork should serve as an indication that a fire inspection will be performed in the very near future.John Hayes did speak with FM Weliver about scheduling the re-inspection of this building/business at 10:00 a.m.on 8/18/2022.John Hayes did not show up for the re-inspection of the business/building at that time, nor did he call to cancel or re-schedule. Building Official Greg Ahlhelm and Commercial Building Inspector Jeff Siebel were both present for the re-inspection of this business/building and can verify the no-call/no-show of John Hayes.-Brock Weliver 8/18/2022 JEFF SIEBEL-08/18/2022 @ 14:48 re-inspection fee$100 Printed on 08/18/2022 @ 14:50 Page 2 of 3 REINSPECTION DATE 09/08/2022 CONTACT SIGNATURE Thomas Hernadus Signed on:08/18/2022 @ 14:49 INSPECTOR SIGNATURE JEFF SIEBEL Signed on:08/18/2022 @ 14:49 QUESTIONS ABOUT YOUR INSPECTION?
