Approved covers shall be provided for all switch and outlet boxes.
Corrective action, replace/install cover plates on electrical outlets
throughout building, mostly on main floor,and replace/install the
cover plate on the breaker panel in the basement.

2 904.12.5 Portable fire extinguishers for commercial cooking
equipment. Portable fire extinguishers shall be provided within a
30-foot distance of travel from commercial-type cooking equipment.
Cooking equipment involving solid fuels or vegetable or animal oils
and fats shall be protected by a Class K rated portable extinguisher
in accordance with Section 904.12.5.1 or 904.12.5.2, as applicable.

3 906.7 Hangers and brackets. Hand-held portable fire extinguishers,
not housed in cabinets, shall be installed on the hangers or
brackets supplied. Hangers or brackets shall be securely anchored
to the mounting surface in accordance with the manufacturer's
installation instructions.

4 304.1 Waste accumulation prohibited. Combustible waste material
creating a fire hazard shall not be allowed to accumulate in
buildings or structures or upon premises.
