Extension cords and flexible cords shall not be a substitute for permanent wiring. Extension cords and flexible cords shall not be affixed to structures, extended through walls, ceilings or floors, or under doors or floor coverings, nor shall such cords be subject to environmental damage or physical impact. Extension cords shall be used only with portable appliances. *Extension cord with a multiplug adapter was found to be in use in room 421 *Corrective action, remove all extension cords being used for permanent wiring in the entire building 3 2015 IFC Building Services and Systems Section 605 Electrical Equipment, Wiring and Hazards 605.6 Unapproved conditions. Open junction boxes and open- wiring splices shall be prohibited.Approved covers shall be provided for all switch and electrical outlet boxes. *Open junction box was found above room 210 in the main hallway *Corrective action, replace junction box cover *There is a security camera wire hanging down by room 215, repair/replace the camera/wires 4 2015 IFC Fire and Smoke Protection Features Section 703 Fire- Resistance-Rated Construction 703.2.3 Door operation. Swinging fire doors shall close from the full-open position and latch automatically.The door closer shall exert enough force to close and latch the door from any partially open position. *Numerous doors to rooms were found to not self close and latch properly *Corrective action, ensure that all rated fire doors operate properly, doors found were, but not limited to, rooms 501, 517, 524, 525,412, 327, 325, 303,229,224, 222, 221 5 2015 IFC Fire Protection Systems Section 901 General 901.4.6 Pump riser room size. Where provided,fire pump and automatic sprinkler system riser rooms shall be designed with adequate space for all equipment necessary for the installation, as defined by the manufacturer,with sufficient working space around the stationary equipment. Clearances around equipment to elements of permanent construction, including other installed equipment and appliances, shall be sufficient to allow inspection, service, repair or replacement without removing such elements of permanent construction or disabling the function of a required fire- resistance-rated assembly. Fire pump and automatic sprinkler system riser rooms shall be provided with a door(s)and an unobstructed passageway large enough to allow removal of the largest piece of equipment. *Items were being stored in the Sprinkler Riser space *Corrective action, remove all storage from around Sprinkler Riser area, ensure that this area is kept clear at all times Ref# CODE Violation &Location Corrected 6 2015 IFC Fire Protection Systems Section 901 General 901.6 Inspection, testing and maintenance. Fire detection,alarm, and extinguishing systems, mechanical smoke exhaust systems, and smoke and heat vents shall be maintained in an operative condition at all times, and shall be replaced or repaired where defective. Nonrequired fire protection systems and equipment shall be inspected,tested and maintained or removed. *Annual inspections were being performed when the building was being inspected, please provide current annual report of all systems *Numerous Smoke Detectors were found to be out when tested *Corrective action, repair/replace smoke detectors, ensure that all Smoke Detectors in the building are working at all times **Smoke Detectors found to be out when tested are, but not limited to rooms 515, 516 (LR), 517 both, 521 (LR), 525 both,422 both, 421 both,416(LR),415(LR),412(BR), 303(LR), 312 (LR), 320 (BR), 327 both, 328, 223, 222, 221,212, 203 Hallway smoke by 312 LR=Living Room, BR=Bedroom 7 2015 IFC Fire Protection Systems Section 901 General 901.4 Installation. Fire protection systems shall be maintained in accordance with the original installation standards for that system. Required systems shall be extended, altered or augmented as necessary to maintain and continue protection where the building is altered, remodeled or added to.Alterations to fire protection systems shall be done in accordance with applicable standards. *Rooms were found to be under construction at the time of inspection, please ensure that all Fire and Life Safety Systems are being maintained as initially installed **Rooms include 505, 507, 510, 512, 515, 529,407 *Ceiling tiles are missing from the maintenance storage unit.The ceiling is part of the Sprinkler System, missing ceiling tiles may allow smoke and fire to bypass the sprinkler deflector heads and rendering the sprinkler system ineffective. **Corrective action, replace missing ceiling tiles 8 2015 IFC Means of Egress Section 1008 Means of Egress Illumination 1008.2 Illumination required.The means of egress serving a room or space shall be illuminated at all times that the room or space is occupied. *Egress lights were found to be out when tested **Corrective Action, replace/repair egress lights, lights found to be out are egress light between 4&5 by 528 and egress light between 2&3 by 305 9 2015 IFC Means of Egress Section 1013 Exit Signs 1013.6.3 Power source. Exit signs shall be illuminated at all times. To ensure continued illumination for a duration of not less than 90 minutes in case of primary power loss,the sign illumination means shall be connected to an emergency power system provided from storage batteries, unit equipment or an on-site generator.
