Document the date,time, personnel performing maintenance and signature. Maintenance/cleaning may be done by hotel personnel. Comments: Adequate time was given to have these violations corrected. On a previous visit around June 4th,the property owner asked for another 2 weeks to have all of the violations fixed/repaired.This time was given. Upon reinspection on June 19th, violations were still not repaired/fixed. Reinspection fees will now be assessed for future reinspections.A follow up reinspection will be made July 3 2018.
