1 2015 IFC: Section 703.2 Opening Protectives: Opening protectives
shall be maintained in an operative condition at all times. Several
fire rated doors in the 2nd floor conference room were found to be
wedged open. Several other doors were found to be wedged open
as well. Fire rated doors are self-closing and need to either remain
closed or be provided with a hold-open device that will allow them
to close when the fire alarm sounds.
2 2015 IFC:Section 605.1 Abatement of Electrical Hazards:2nd floor
conference kitchen area had a microwave plugged into a portable
power tap.This appliance should be plugged directly into a
permanent wall outlet. This violation was corrected on site.

3 2015 IFC: Section 901.6 Inspection,testing, and maintenance: Fire
extinguishing systems shall be maintained in an operative condition
at all times.A few dirty(loaded)sprinkler heads were found in the
dentistry area. Heads should be cleaned whenever they become
covered with dust, lint,etc.. Consult with your sprinkler contractor
for best practice to clean off heads.
4 2015 IFC: Section 605.4 Multiplug adapters: Multiplug adapters are
prohibited. Several multiplug adapters(3 outlet strip plugged into
one receptacle)were found in the emergency care area and Exam
Room#4.These adapters were removed by maintenance staff.
These violations were corrected on site.
Comments: At this time a re-inspection will not be scheduled. However, maintenance was
advised to work with staff to educate them to no longer use door wedges or
multiplug adapters. Please feel free to contact me with any questions or concerns.
A spot re-inspection may take place later in the year. Thank you for your time and
cooperation.
