1 2015 IFC: Section 906.2- Portable fire extinguisher was found to be
out of date. Extinguisher was tagged March of 2016.Corrective
action: Provide annual servicing for fire extinguisher.
2 2015 IFC: Section 605.1-Portable refrigerator was found to be
plugged into an extension cord in Dr. Reade's office.Appliance
should be plugged directly into permanent wall outlet. Corrective
action: Relocate refrigerator so that it can be plugged into wall
outlet or add additional permanent outlets.

3 2015 IFC: Section 703.2.3-Both sets of rated fire doors in the
alliance space did not automatically latch when closed. One was a
door leading to the restrooms and the other was the main
entrance. Corrective action: Provide repair so that the rated doors
will automatically close at latch.
4 2015 IFC: Section 605.10-Several portable electric space heaters
were found to be in use on the day of inspection.All of the space
heaters in operation were found to be within 3 feet of combustible
materials(wastebaskets, papers, boxes,desks). Portable space
heaters cannot be used within 3 feet of combustible items.
Corrective action: Please remove space heaters or provide
documentation of the heaters listings for review.
