FAIL 5003.5-Hazard identification Unless otherwise exempted by the fire code official,visible hazard identification signs
signs as specified in NFPA 704 for the specific material contained shall be placed on
stationary containers and above-ground tanks and at entrances to locations where
hazardous materials are stored,dispensed, used or handled in quantities requiring a
permit and at specific entrances and locations designated by the fire code official.
Inspector:JEFF SIEBEL-Comments:re-install fuel tank storage signage.
FAIL 604.2.12-Means of egress Emergency power shall be provided for means of egress illumination in accordance
illumination with Sections 1008.3 and 1104.5.1.

FAIL 604.2.5-Exit signs Emergency power shall be provided for exit signs as required in Section 1013.6.3.The
system shall be capable of powering the required load for a duration of not less than
90 minutes.

main lower level exit.B.men's locker room inside exit door by shoe shine.C. by

FAIL 604.4.2-Records Records of the inspection,testing and maintenance of emergency and standby
power systems shall include the date of service, name of the servicing technician,a
summary of conditions noted and a detailed description of any conditions requiring
correction and what corrective action was taken.Such records shall be maintained.

emergency egress and exit lights with all discrepancies corrected with a signature
and date.

FAIL 605.3-Working space and A working space of not less than 30 inches(762 mm)in width,36 inches(914 mm)in
clearance depth and 78 inches(1981 mm)in height shall be provided in front of electrical
service equipment.Where the electrical service equipment is wider than 30 inches
(762 mm),the working space shall be not less than the width of the equipment.
Storage of materials shall not be located within the designated working space.
Exceptions: 1.Where other dimensions are required or allowed by NFPA 70.2.Access
openings into attics or under-floor areas which provide a minimum clear opening of
22 inches(559 mm)by 30 inches(762 mm).
Inspector:JEFF SIEBEL-Comments: Remove obstructions to electrical panel in
upstairs coat room by restrooms. Maintain a minimum of 36"clearance to and
around electrical panel.
FAIL 605.4-Multiplug adapters Multiplug adapters,such as cube adapters, unfused plug strips or any other device
not complying with NFPA 70 shall be prohibited.
Inspector:JEFF SIEBEL-Comments:Remove multi-plug adapters and extension cords
at ceiling projectors:A. Pinehust Room. B.Inverness Room.
FAIL 605.4.2-Power supply Relocatable power taps shall be directly connected to a permanently installed
receptacle.
Inspector:JEFF SIEBEL-Comments:remove power tap from other power tap(daisy
chaining)and plug directly into a permanent wall outlet behind counter in pro shop.
FAIL 703.1 -Maintenance The required fire-resistance rating of fire-resistance-rated construction,including,
but not limited to,walls,firestops,shaft enclosures,partitions,smoke barriers,
floors,fire-resistive coatings and sprayed fire-resistant materials applied to
structural members and fire-resistant joint systems,shall be maintained.Such
elements shall be visually inspected by the owner annually and properly repaired,
restored or replaced where damaged,altered, breached or penetrated. Records of
inspections and repairs shall be maintained.Where concealed,such elements shall
not be required to be visually inspected by the owner unless the concealed space is
accessible by the removal or movement of a panel,access door,ceiling tile or similar
movable entry to the space.Openings made therein for the passage of pipes,
electrical conduit,wires,ducts,air transfer openings and holes made for any reason
shall be protected with approved methods capable of resisting the passage of smoke
and fire.Openings through fire-resistance-rated assemblies shall be protected by
self-or automatic-closing doors of approved construction meeting the fire protection
requirements for the assembly.

FAIL 906.8-Cabinets Cabinets used to house portable fire extinguishers shall not be locked. Exception: 1.
Where portable fire extinguishers subject to malicious use or damage are provided
with a means of ready access.2. In Group 1-3 occupancies and in mental health areas
in Group 1-2 occupancies,access to portable fire extinguishers shall be permitted to
be locked or to be located in staff locations provided the staff has keys.
Inspector:JEFF SIEBEL-Comments:repair fire extinguisher cabinet and install a
striking tool on a chain attached to cabinet at fuel pump station.
FAIL Chapter 14-Inspection, Fire alarm system tested at least annually by licensed contractor
testing,and maintenance
Inspector:JEFF SIEBEL-Comments:provide annual fire alarm inspection report with
all discrepancies corrected.contractor must upload online to the compliance engine.
