STATUS CODE DESCRIPTION
FAIL 315.3.1 -Ceiling clearance Storage shall be maintained 2 feet(610 mm)or more below the ceiling in
nonsprinklered areas of buildings or not less than 18 inches(457 mm)below
sprinkler head deflectors in sprinklered areas of buildings.
Inspector: BROCK WELIVER-Comments:2nd Floor-Custodial Closet. Maintain
storage as described above.This item was fixed during the inspection
FAIL 5303.5.3-Securing Compressed gas containers,cylinders and tanks shall be secured to prevent falling
compressed gas containers, caused by contact,vibration or seismic activity.Securing of compressed gas
cylinders and containers,cylinders and tanks shall be by one of the following methods: 1.Securing
containers,cylinders and tanks to a fixed object with one or more restraints.2.
Securing containers,cylinders and tanks on a cart or other mobile device designed
for the movement of compressed gas containers,cylinders or tanks.3. Nesting of
compressed gas containers,cylinders and tanks at container filling or servicing
facilities or in sellers'warehouses not accessible to the public. Nesting shall be
allowed provided the nested containers,cylinders or tanks,if dislodged,do not
obstruct the required means of egress.4.Securing of compressed gas containers,
cylinders and tanks to or within a rack,framework,cabinet or similar assembly
designed for such use. Exception:Compressed gas containers,cylinders and tanks in
the process of examination,filling,transport or servicing.
Inspector: BROCK WELIVER-Comments: Urgent Care-Room 3,02 bottles were found
loosely stored in this area. Ensure that all bottles,whether full or empty,are properly
stored as described above.
FAIL 604.2.12-Means of egress Emergency power shall be provided for means of egress illumination in accordance
illumination with Sections 1008.3 and 1104.5.1.

as you walk in the entry door,egress light was out when tested

FAIL 605.1 -Abatement of Identified electrical hazards shall be abated. Identified hazardous electrical conditions
electrical hazards in permanent wiring shall be brought to the attention of the responsible code official.
Electrical wiring,devices,appliances and other equipment that is modified or
damaged and constitutes an electrical shock or fire hazard shall not be used.
Inspector: BROCK WELIVER-Comments:2nd Floor-PEDS,Christmas lights were
plugged in to an amplifier.Temporary lighting such as Christmas lights are given
approximately 90 days for display purposes.
FAIL 605.7-Appliances Electrical appliances and fixtures shall be tested and listed in published reports of
inspected electrical equipment by an approved agency and installed and maintained
in accordance with all instructions included as part of such listing.
Inspector: BROCK WELIVER-Comments:2nd Floor-Break Room, microwave was
plugged into a power tap.2nd Floor-Medical Records, mini fridge was plugged into a
power tap. Ensure that all larger appliances with motors and compressors are
plugged directly into wall outlets.
FAIL 609.2-Where required A Type I hood shall be installed at or above all commercial cooking appliances and
domestic cooking appliances used for commercial purposes that produce grease
vapors. Exception:A Type I hood shall not be required for an electric cooking
appliance where an approved testing agency provides documentation that the
appliance effluent contains 5 mg/m^3 or less of grease when tested at an exhaust
flow rate of 500 cfm(0.236 m^3/s)in accordance with UL 710B.
Inspector: BROCK WELIVER-Comments:2nd Floor-Break Room,ensure that no
sautéing or frying is conducted on this stove.If you would like to use the stove for
this purpose,then a Type 1 hood shall be installed as described above.Corrective
Action:provide a sign above the stove at states: No sauteing or frying allowed.
FAIL 703.1 -Maintenance The required fire-resistance rating of fire-resistance-rated construction,including,
but not limited to,walls,firestops,shaft enclosures,partitions,smoke barriers,
floors,fire-resistive coatings and sprayed fire-resistant materials applied to
structural members and fire-resistant joint systems,shall be maintained.Such
elements shall be visually inspected by the owner annually and properly repaired,
restored or replaced where damaged,altered, breached or penetrated. Records of
inspections and repairs shall be maintained.Where concealed,such elements shall
not be required to be visually inspected by the owner unless the concealed space is
accessible by the removal or movement of a panel,access door,ceiling tile or similar
movable entry to the space.Openings made therein for the passage of pipes,
electrical conduit,wires,ducts,air transfer openings and holes made for any reason
shall be protected with approved methods capable of resisting the passage of smoke
and fire.Openings through fire-resistance-rated assemblies shall be protected by
self-or automatic-closing doors of approved construction meeting the fire protection
requirements for the assembly.
Inspector: BROCK WELIVER-Comments: 1st Floor-janitors Closet,penetration in
drywall needs to be fire caulked. 1st Floor-Pharmacy,2 ceiling tiles were missing
near the customer help window.

FAIL 703.2-Opening protectives Opening protectives shall be maintained in an operative condition in accordance with
NFPA 80.Where allowed by the fire code official,the application of field-applied labels
associated with the maintenance of opening protectives shall follow the
requirements of the approved third-party certification organization accredited for
listing the opening protective. Fire doors and smoke barrier doors shall not be
blocked or obstructed,or otherwise made inoperable. Fusible links shall be replaced
promptly whenever fused or damaged. Fire door assemblies shall not be modified.
Inspector: BROCK WELIVER-Comments:2nd Floor-IT Room, rating tags on fire doors
were found to be painted,ensure that the tags are legible.
FAIL 703.2.2-Hold-open devices Hold-open devices and automatic door closers,where provided,shall be maintained.
and closers During the period that such device is out of service for repairs,the door it operates
shall remain in the closed position.
Inspector:BROCK WELIVER-Comments:Discontinue the practice of using door
wedges or objects to prop doors open. Multiple fire rated doors were found to be
propped open. If doors are desired to be left in the open position,then hold-open
devices should be installed that are connected to the fire alarm system.
