FAIL 1.4-F/A acceptance testing- Verify that duct detectors: 1)Shut down the unit they serve upon activation 2)Send
Duct Detectors an audible and visible supervisory signal to the fire alarm panel 3)Detectors
mounted in ducts 10 feet or higher shall be installed with a LED activation light and
remote test switch
Inspector:JEFF SIEBEL-Comments:install duct detectors and provide acceptance
testing by fire marshal.
FAIL 1031.2-Reliability Required exit accesses,exits and exit discharges shall be continuously maintained
free from obstructions or impediments to full instant use in the case of fire or other
emergency where the building area served by the means of egress is occupied.An
exit or exit passageway shall not be used for any purpose that interferes with a
means of egress.
Inspector:JEFF SIEBEL-Comments: Install permanent sign on outside of back exit.
Install permanent red sign with red letters stating"Fire Exit Do Not Block"
FAIL 604.2.12-Means of egress Emergency power shall be provided for means of egress illumination in accordance
illumination with Sections 1008.3 and 1104.5.1.
Inspector:JEFF SIEBEL-Comments:repair egress light in immunization room.
FAIL 604.4.2-Records Records of the inspection,testing and maintenance of emergency and standby
power systems shall include the date of service,name of the servicing technician,a
summary of conditions noted and a detailed description of any conditions requiring
correction and what corrective action was taken.Such records shall be maintained.

lights with all corrections completed with signature and date.

Extension cords and flexible cords shall not be affixed to structures,extended
through walls,ceilings or floors,or under doors or floor coverings,nor shall such
cords be subject to environmental damage or physical impact. Extension cords shall
be used only with portable appliances.
