All Bidders submitting a bid on the above contract shall carefully read this Addendum and give it
consideration in the preparation of their bid.
TO ALL BIDDERS ON THE ABOVE PROJECT:
I. The FORM OF BID OR PROPOSAL for this project is hereby revised as follows:
A. On the FORM OF BID OR PROPOSAL in Division II, Page 5 of 7, Bid Items 2 and 5
through 20 have incorrect estimated quantities.
Bidder shall use the revised FORM OF BID OR PROPOSAL page 5 of 7 included as part of
this Addendum No. 3.
II. The City of Waterloo Engineering Department has received a CENTURY LINK
RELOCATION PLAN from Terra Technologies for this project. As part of this
Addendum No. 3, the plan (4 MB in size) is being sent by email to any plan holder that
has provided an email address. Hard copies are available upon request. (319-291-
4312)
Any revisions to any of the Contract Documents made by this Addendum shall be considered as
the same revision to any and all related areas of the Contract Documents not specifically called out
in this Addendum.
The bidder shall acknowledge receipt of this Addendum by inserting the number and date in the
spaces pro de •n theForm of Bid or Proposal, Item 9, Page 7 of 7.
