Ward I Waterloo Leisure Services recently sent you a bid request for modifications
CAROLYN and updates to Sullivan Park basketball courts located near corner of East 4th COLE and Adams St. Under the SPECIFICATIONS section there is a change in
Ward 2 information being provided. We are notifying you of this Addendum to that
section. The additional information is the removal of density tests/core

HART Density Tests on Asphalt Concrete in Place:

The Contractor shall provide density testing on the asphalt concrete by
RON core sample from project per S.U.D.A.S. Section 7020, 3.06. The
WELPER contractor will cut cores and test per IDOT and SUDAS specifications.
Ward 5
BOB Leisure Services is no longer requiring density testing or core sampling.

The information contained in this Addendum modifies, supplements or replaces
information contained in the Project Manual and on the Drawings and is hereby made a
part of the Contract Documents.
This addendum contains one (1) page.
Please acknowledge receipt of this addendum on Form of Bid Page 2 prior to
submittal of your bid as well as signing and faxing the Faxed Addendum Receipt. If
you have any questions, please call our office.
PROJECT MANUAL ITEM
1. Tree and Stump removal required for the construction of the services shall be
incidental to the project and paid as part of Mobilization.
2. PVC pipe shall be used for Sanitary Sewer Service. References of RCP shall be
changed to PVC SDR 23.5. References to RCP include, but are not limited
to:
a. "Notice of Public Hearing" section, page 2 within Scope of Work.
b. "Form of Bid or Proposal" section, page 1 within the bid items tabulation.
c. "Items of Potential MBE/WBE Participation" section, page 1 within
descriptions.
d. "Special Provisions" section, pages 2-4 within item 4 and 5 descriptions.
PVC Sanitary Sewer Service shall comply with SUDAS Sanitary Sewer Service
specifications, section 4010, 2.04 C.
