CAROLYN EQUIPMENT UPGRADE, CHANGE ORDER NO. 1

The scope of work as outlined in the bid document as prepared by Gilmor and
HAROLD Doyle for Phase II of the Five Sullivan Brothers Convention Center mechanical

Ward 3 equipment upgrade is about ninety (90) percent complete. However, during the
process of replacing the new listed equipment, additional valves have shown
JOHN A. themselves to be in a failed or near-failed condition.

Johnson Controls has submitted Change Order No. 1 for the replacement of all
RON additional defective valves and the rectifying of several air handler conditions that
WELPER were not spelled out in the document prepared by Gilmor and Doyle (i.e., air handler
ward expansion joints, air handler door problems, etc.). The total cost of the additional
BOB work, labor and material will be $104,890. Do not have heart failure! There are
GREENWOOD four (4) items in the document that need immediate attention before we get into the
At-Large heating season. Hot water pumps 1, 2 and 3 and heat exchanger butterfly isolation
ERIC valves and triple-duty valves that are leaking internally need replacing.
GUNDERSON
At-Large I recommend a change order for $30,650 to be issued to Johnson Controls for these
repairs.
The additional valve replacements are on the chilled-water side of the system. My
staff will work with those valves through the winter months so that by the next
cooling season, the complete system will be automated and operational.
I'm requesting authorization to send to Council Change Order No. 1 from JCI in the
amount of$30,650 for the selected items as outlined in the Johnson Controls
document dated October 3, 2006.
