We hereby propose to furnish all the materials, equipment and perform all labor for the completion of:
removal & disposal of approximately all acm as described in asbestos report done on
12-22-06.
Proposal includes: (1 ) OSHA required personal & excursion air monitoring, (2) work
performed in accordance with state & federal regulations, (3) clearance air monitoring
The Iowa Division of Labor & Environmental Protection Agency require a ten (10)
working day notification before work can commence.
WE PROPOSE to furnish material and labor, complete in accordance with the above specifications for the
sum of: One Hundred Nine Thousand Four Hundred Thirty Dollars ($109,430.00*)
Terms: Net 30 Days! All accounts subject to 1.5% monthly charge after 30 days.
All work is guaranteed to be as specified. All work to be completed in a workmanlike manner Respectfully Submitted,
according to specified practices. Any alteration or deviation from the above specifications
involving extra costs will be executed only upon written orders and will become an extra charge
above the estimate. All agreements contingent upon strikes,accidents or delays beyond our ADVANCED ENVIRONMENTAL
control. Owner to carry fire,tornado,and other necessary insurance. We carry general liability
insurance and our workers are fully covered by workers compensation insurance. TESTING AND ABATEMENT, INC.
NOTE: This proposal may be withdrawn by us if not
accepted within 30 days.
