We hereby propose to furnish all the materials, equipment and perform all labor for the completion of:
removal and disposal of black tar on rolled roofing seams and edges, window glazing, and
caulking around the SE door at the above referenced project.
Proposal Includes: (1) daily OSHA required personal & excursion air monitoring.
State of Iowa requires a ten (10) working day notification by the contractor prior to
commencement of work.
WE PROPOSE to furnish material and labor, complete in accordance with the above specifications for the
sum of: One Thousand Four Hundred Fifty Dollars ($1,450.00*)
Terms: Net 30 Days! All accounts subject to 1.5% monthly charge after 30 days.
All work is guaranteed to be as specified. All work to be completed in a workmanlike manner Respectfully Submitted,
according to specified practices. Any alteration or deviation from the above specifications
involving extra costs will be executed only upon written orders and will become an extra charge
above the estimate. All agreements contingent upon strikes,accidents or delays beyond our
control. Owner to carry fire,tornado,and other necessary insurance. We carry general liability ADVANCED ENVIRONMENTAL
insurance and our workers are fully covered by workers compensation insurance. TES NG AND ABATEMENT, INC.
