We hereby propose to furnish all the materials, equipment and perform all labor for the completion of:
removal & disposal of the asbestos listed in the inspection reports for the above referenced
project at these addresses:
218-220 W 2nd Street:...$2250 (540 sq.ft. F.T., duct paper,stair tread,windows)
222-224 W 2nd Street:...$1500 (Duct paper, 24 sq.ft. counter, 120 sq.ft. linoleum)
118 E 10th Street- $1500 (460 sq.ft. F.T, 5 sq.ft.lino, duct paper,windows)
1 16-1 18 E 8th Street- $1500 (400 sq.ft. linoleum,duct paper,roof tar)
1218 Sycamore Street...$3100 (540 sq. ft.texture,380 sq.ff.flooring-floor tile &linoleum, 100 sq.ft sheetrock joint
compound)
Proposal Includes: (1) daily OSHA required personal & excursion air monitoring
State of Iowa requires a ten (10) working day notification by the contractor prior to
commencement of work.
WE PROPOSE to furnish material and labor, complete in accordance with the above specifications for the
sum of: Nine Thousand Eight Hundred Fifty Dollars ($9,850.00*)
Terms: Net 30 Days! All accounts subject to 1.5% monthly charge after 30 days.
All work is guaranteed to be as specified. All work to be completed in a workmanlike manner Respectfully Submitted,
according to specified practices. Any alteration or deviation from the above specifications
involving extra costs will be executed only upon written orders and will become an extra charge
above the estimate. All agreements contingent upon strikes,accidents or delays beyond our ADVANCED ENVIRONMENTAL
control. Owner to carry fire,tornado,and other necessary insurance. We carry general liability
insurance and our workers are fully covered by workers compensation insurance. TESTING AND ABATEMENT, INC.
NOTE: This proposal may be withdrawn by us if not
accepted within 30 days.
