Box 295 Jefferson, Iowa 50129-0295 RE: Bid for Removal of Asbestos Contract No. 13 9-AR-03-21-11 EDI Dear Mr. Nielson: The Community Development Board is soliciting bids for the removal of asbestos from six (6) structures located in the City of Waterloo, Iowa: 706 W. 1st Street 915-17 Lafayette Street 919 Lafayette Street 1013 Lafayette Street 1021 Lafayette Street Alternate: 927 Lafayette Street We require an itemized quote that is to be totaled. This will also require immediate DNR notification. 1. Bid must be SIGNED and bear the official title of the person signing. Bids must be accompanied by a 5% Bid bond, certified funds or corporate surety. 2. Bidders must be prepared to proceed as soon as possible. 3. Bid must be submitted SEALED in an envelope addressed to: Suzy Schares CITY CLERK 715 Mulberry Street Waterloo, Iowa 50703 and must be marked in the lower left-hand corner as follows: BID FOR ASBESTOS REMOVAL AND DISPOSAL 139-AR-03-21-11 EDI TO BE OPENED: 5:30 P.M. March 14,2011 March 4, 2011 Page 2 3. Sufficient time should be allowed for mailed bids to be delivered by normal postal operation. Late bids will not be considered. Bids must be received by 3:30 p.m., March 14, 2011 in the City Clerk's office. Bids will be opened at the City Council regular meeting of Finance/Building and Grounds Committees at 5:30 p.m. 4. Asbestos abatement shall be completed within Ten (10)working days of Notice to Proceed(NTP). TIME IS OF THE ESSENCE and default requirements will be strictly adhered to. Failure to complete the project as set forth herein, shall constitute a material default and the City may immediately hire another contractor to complete work and deduct any costs associated from the original contract price associated with the contract substitution. Execution of this Contract requires flexibility and the ability to perform under extraordinary conditions. 5. Your bid proposal shall include spaces for authorized signature(s) of the City's representative,to be signed if the lowest responsible bid. Submit proposals in duplicate. Bids to be awarded at the City Council Meeting March 21, 2011. 6. Enclosed is a list of asbestos items for each property. Remove only areas indicated on the report as having asbestos in the structure. 7. Please pay attention to the Asbestos Inspection Report's Notes & Recommendations. 8. Proposals should include (1) OSHA required personal and excursion air monitoring, and (2)work performed in accordance with EPA-EHERA,NESHAP, Iowa DNR and Iowa DNR regulations.
