O.Box 295 MEMBERS Jefferson, Iowa 50129-0295 RE: Asbestos Survey DAVID Bid Invitation Project Number: 139-AS-12-13-10-EDI JONES Ward 1 Dear Mr.Nielson: CAROLYN COLE The City of Waterloo is soliciting bids from licensed and approved Asbestos Inspection Ward 2 Contractors to conduct asbestos inspections and analysis and report on its findings for all Six(6) structures for possible asbestos containing materials. Your firm is invited to submit HAROLD GETTY a bid to conduct an approved asbestos survey of the following structures in Waterloo, Ward 3 Iowa: 1. 915-17 Lafayette Street,Waterloo,Iowa QUENTIN M. RON 5. 1021 Lafayette Street,Waterloo,Iowa WELPER6. 706 W. 1st Street,Waterloo,Iowa Ward 5 5 BOB An adequate number of samples should be taken to identify possible asbestos-containing GREENWOOD materials. At-Large STEVE We are requiring an estimate for the referenced addresses, and the lowest responsible SCHMITT bidder to indicate on the report the samples taken, the room location and its area, the type At-Large of asbestos-containing material (ACM), the percentage and quantity, and other pertinent information in regard to the recognized requirements of a survey. Also, include on your proposal a line for an acceptance signature if your firm is the lowest and most responsible bid. 1. Bid must be SIGNED, and bear the official title of the person signing. 2. The Failure to submit a sealed bid will result in a disqualification of your bid. 4. All Bids must be accompanied with a 5% BID BOND. Bid security must be in the form of a cashier's check, a certified check or a certified share draft, or it also can be provided in the form of a bid bond with corporate surety. A bid submitted without the required bid security must be rejected. 5. Sufficient time should be allowed for mailed bids to be delivered by normal Postal operation. Late bids will not be considered. Faxed bids will not be accepted. Bids must be received by 4:00 PM CDT on December 06,2010 at the address shown above. Bid will be Awarded on December 13, 2010 in Council Chambers. The City of Waterloo will review bids and the lowest responsible bidder will be given notice to proceed after appropriate approvals are obtained. 6. Copies of properties descriptions may be obtained from the Black Hawk County website: www.co.black-hawk.ia.us. A site location Map is enclosed. 7. The successful bidder must be prepared to proceed and complete the project including submitting a Report/Survey within Fifteen (15) days of the date of the Notice to Proceed. 8. All bidders must be licensed and registered with Iowa Northland Regional Council of Government, Iowa Contractors Registration database, and maintain appropriate State and Federal certifications and licenses(s) and trained staff and be able to present credentialing within 24 hours of such request by staff or others. 9. Pursuant to OMB Circular A-110 Procurement Standards, in order to ensure objective contractor performance and eliminate unfair competition, contractors that develop or draft specifications, requirements, statements of work, invitations for bids and/or requests for proposals shall be excluded from competing for such procurements. Accordingly,the successful bidder for this Asbestos Removal Project will not be permitted to bid on the asbestos removal portion of this project because of potential competitive advantages. All bids must encompass total costs, including travel, samples, report and shipping and handling, the contractor is responsible for complete inspection of the premises prior to submitting bid. 11. The contractor indemnifies the City from any undertaking including the Site Visitation. 12. The bid must also include the estimated number of samples taken per structure, itemized cost for each structure, and a base bid. 13. In submitting this bid, the bidder understand that the City of Waterloo reserves the right to reject any and all bids and/or to select or reject any bid that is in the City's best interest, or waive minor irregularities if in the City's best interest.
