Gentlemen: 1. The undersigned, being a Corporation existing under the laws of the State of Iowa, a Partnership consisting of the following partners: an Individual doing business as having familiarized (himself), (themselves), (its with the existing conditions on the Project Area affecting the cost of the work and with the Contract Documents of Group 142-D- GO-02-24-06-VGM which includes Invitation for Bids, Instructions to Bidders, the Form of Bid, Form of Contract (or agreement), Form of Non-Collusion Affidavit, Non-Segregated Facilities, Addenda #1 General Conditions, Special Conditions, Technical Specifications Drawings (as listed in the Schedule of Drawings), and on file in the office of the Community Development Board, hereby proposes to furnish all supervision,technical personnel, labor, materials, machinery,tools, equipment and services including utility and transportation services and to start demolition within seven(7) days of Notice to Proceed, complete all work required for the Demolition and Site Clearance for City of Waterloo General Obligation Bond Demolition Project, within 20 calendar days of Notice to Proceed, all in accordance with above listed documents. In submitting this bid, the bidder understands that the right is reserved by the City of Waterloo acting through its Community Development Department to reject any and all bids. If written notice of the acceptance of this bid is mailed, telegraphed or delivered to the undersigned within two (2) days after the opening thereof, or at any time thereafter before this bid is withdrawn,the undersigned agrees to execute and deliver an Agreement in the prescribed form and furnish the required bond within ONE (1) day after the agreement is presented to him/her for signature. Attached hereto is a Non-Collusion Affidavit and a Non-Segregated Affidavit in proof that the undersigned has not colluded and does not have segregated facilities with any person in respect to this bid or any other bid for the Contract for which this bid is submitted. 4. The bidder agrees that a breach of this certification will be a violation of the Equal Opportunity Clause in any Contract resulting from acceptance of this bid. As used in this certification, the term segregated facilities means any waiting room, work areas, restrooms and washrooms, restaurants and other eating areas, time clocks, locker rooms and other storage or dressing areas, parking lots, drinking fountains, recreation or entertainment areas, transportation, and housing facilities provided for employees which are segregated by explicit directive or are in fact segregated on the basis of race, color, religion, or national origin, because of habit, local custom, or otherwise.
