Obligation Bond Funds Demolition Contract Group No. 144-D-GO-08-18-06.

GREENWOOD Bids will be opened in Council Chambers on Friday, August 18, 2006 at 3:30 p.m. and
At-Large awarded to the lowest responsible bidder by the Finance Committee on Monday, August 21,
2006. The actual work is anticipated to begin on August 22, 2006 with the transmittal of the
ERIC IDNR notification, with completion by September 23, 2006, weather permitting.
GUNDERSON
At-Large There will be water and sewer cut-offs, as required by City Codes and regulations, as part of
the demolition contract. The various City departments concerned with this work will be
participating in the supervision of the demolition work.
It is the contractor's responsibility to secure proper permits, obtain proper inspections and
signoffs, and legal compliance for this project. Failure to do so could result remedies,
including cancellation of contract, nonpayment, fines, suspensions or debarment.

ADDENDUM NO. 1
1 . For EACH Structure, the CONTRACTOR shall
remove all basement, foundation, and footings
in their entirety.
2. FENCE/TREE REMOVAL AND GRUBBING
At 738 Mobile St.
CONTRACTOR shall remove the fence and all
volunteer trees and shrubs along the fence line.
All other trees designated for removal are marked
with white paint. To insure clarity and consistency
in quotes a MANDATORY SITE VISIT is
scheduled for Tuesday, August 15, 2006 at 9 a.m.,
738 Mobile St., Waterloo, IA.

Nancy, each of the 3 companies submitting a bid received this addendum, 2 chose to ignore it, and didn't attend
the Mandatory Site Visit. They were Frickson and Kayser, accordingly, they are disqualified and Lehman, though
his bid is highest, is the apparent low bidder.

To the City of Waterloo, Iowa, acting through its Community Development Department;
Carnegie Annex, Suite 202, 620 Mulberry Street, Waterloo, Iowa 50703.
Gentlemen:
1. The undersigned, being a Corporation existing under the laws of the State of Iowa, a
Partnership consisting of the following partners:
an Individual doing business as
having familiarized (himself), (themselves), (itself) with the existing conditions on the Project
Area affecting the cost of the work and with the Contract Documents of Group 144-D-GO-08-
18-06 which includes Invitation for Bids, Instructions to Bidders, the Form of Bid, Form of
Contract (or agreement), Form of Non-Collusion Affidavit, Non-Segregated Facilities, Addenda
General Conditions, Special Conditions, Technical Specifications Drawings(as listed in the
Schedule of Drawings), and on file in the office of the Community Development Board, hereby
proposes to furnish all supervision, technical personnel, labor, materials, machinery, tools,
equipment and services including utility and transportation services and to start demolition within
seven (7) days of Notice to Proceed, complete all work required for the Demolition and Site
Clearance for City of Waterloo General Obligation Bond Demolition Project, within 20 calendar
days of Notice to Proceed, all in accordance with above listed documents.

2. In submitting this bid, the bidder understands that the right is reserved by the City of Waterloo
acting through its Community Development Department to reject any and all bids. If written
notice of the acceptance of this bid is mailed, telegraphed or delivered to the undersigned
within two (2) days after the opening thereof, or at any time thereafter before this bid is
withdrawn, the undersigned agrees to execute and deliver an Agreement in the prescribed
form and furnish the required bond within ONE (1) day after the agreement is presented to
him/her for signature.

6. The bidder certified that he/she does not maintain or provide for his/her employees any
segregated facilities at any of his establishments, and that he does not permit his employees to
perform their services at any location, under his control, where segregated facilities are
maintained. The bidder agrees that a breach of this certification will be a violation of the Equal
Opportunity Clause in any Contract resulting from acceptance of this bid. As used in this
certification, the term segregated facilities means any waiting room, work areas, restrooms and
washrooms, restaurants and other eating areas, time clocks, locker rooms and other storage or
