We have been advised by the Black Hawk County Health Department that a child with an
CAROLYN elevated blood lead level lives at the above referenced property. The property was inspected
COL E and found to have deteriorating lead paint conditions,therefore in our review of this property,
Wml 2 it was determined that the best course of action was to perform a lead abatement and house
rehabilitation. We obtained bids to abate the lead issues and rehabilitate the home from the

Ward After working with the low bidder, we reduced this cost to $39,858.00, including the building
permit. Therefore, we are requesting$4,759.00 of G. O. Bond funding to assist in the repair
SOD of this property with the remaining funds coming from both the Lead Grant totaling
GREENWOOD $10,000.00, and from CDBG Rehabilitation Program funds totaling$24,999.00.
We request Council authorization to award the bid for lead abatement and housing
ErtC rehabilitation to SJ Construction, in the amount of$39,858.00. Contracts will be executed on
GUNDERSON
At-Lance- behalf of the City utilizing the above designated G.O. Bond, Lead Grant, and CDBG funds.
