QUENTIN NI HART The City of Waterloo is seeking bids for the replacement of carpet, VCT and vinyl lEnd 4 base in the Human Resource Department, first floor, Carnegie Annex and the RAN Planning and Zoning, Engineering and Building Departments at City Hall. Bidder shall supply three percent (3%) of estimated carpet square footage as extra to owner for future replacement needs. 8. Bidder shall be responsible to dispose of old carpet and base. 9. Bidder shall be responsible to confirm all measurements of carpet area and tile area. 10.Bidder shall be responsible for moving furniture, file cabinets, etc. to lay carpet and install tile. 11.Bidder agrees to repair or replace, without cost to the owner, any defects due to workmanship or material for a period of one (1) year after final acceptance. 12.The bidder will be responsible to seal and apply two (2) coats oY manufacturer-approved wax on the VCT. BID REQUIREMENTS: 1. A mandatory pre-bid meeting will be held Thursday, May 14, 2009, at 1:00 P.M. at: City Hall 715 Mulberry Street Waterloo, IA 50703 First Floor Conference Room 2. Bid submittal deadline shall be May 26, 2009, on or before 3:00 P.M. 3. Bid envelope shall be clearly marked "CARPET REPLACEMENT /HUMAN RESOURCES OFFICE/CARPET AND TILE REPLACEMENT SECOND FLOOR CITY HALL." 4. Bids shall be submitted to: City Clerk City of Waterloo, City Hall 715 Mulberry Street Waterloo, IA 50703 5. Bids will be opened May 26, 2009 at 5:30 P.M. at the regular session of the City Council. Page 3 6. Bid will be awarded June 1, 2009, to the lowest responsible bidder. 7. Notice to proceed will be given June 2, 2009. 8. Completion of project shall be September 30, 2009. Bonds: 1. A five percent (5%) bid bond shall accompany the submitted bid in a separate envelope clearly marked "Bid Bond, Flooring Project". 2. A Payment/Performance bond for the full amount of the bid price will be required for this project. Payment: 1. All payments will be subject to a five percent (5%) retainage. 2. Pay request will be honored for the amount of material in place. 3. Payment will be made within thirty (30) days after pay request is submitted. Attachments: I have attached two (2) sheets showing the square footage of all areas. It is still the contractor's responsibility to check all measurements.
